Scapulohumeral periarthritis, which is also called Frozen shoulder, Adhesive capsulitis or painful stiff shoulder, is characterised by spontaneous onset of shoulder pain accompanied by progressive stiffness and disability. It is usually self-limiting but often has a prolonged course. It's the chronic soft tissue injury that is common in clinic. It has a seriously influence on people's health and normal life quality.The acupotomology is invented by Professor Zhu Hanzhang on the foundation of the small needle knife therapy. It is a new Chinese medical treatment, which is under the guide of Chinese medical theory and also absorbs surgical intervention theory. NOW, it has developed to a new science with intellectual property rights.A lot of clinical reports confirm that, acupotomy to treat the frozen shoulder syndrome has the advantage of rapidness,short time and efficiency. However, the focus of the current research is still on the summary of clinical effectiveness, but lack of in-depth study on the mechanism of acupotom's effect at different state.ObjectiveFrozen shoulder has three pathological stages named pain period, adhesion period and remission period respectively. In this study, the intervention effects of acupotomy were compared among pain period intervention only, adhesion period intervention only and two periods joint intervention (pain period and adhesion period) to the scapulohumeral periarthritis rabbit model. This study will lay the experimental theoretical basement for seeking the rational acupotomy intervention time in clinic. By studying the inflammatory cells (5-HT, IL-1β, IL-10, PGE2) in the blood serum and the muscles around the shoulder of the scapulohumeral periarthritis rabbit model, with the temperature as the auxiliary index to reveal the acupotomology mechanism at different periods of scapulohumeral periarthritis, by which the study can provide scientific experiment basis for the clinical effect to treat the scapulohumeral periarthritis by lysis of acupotomology intervention.MethodsChoose 48 healthy male New Zealand rabbits and divide them randomly into 6 groups:Normal group, model group, electroacupuncture group, acupotomy group A (pain stage intervention only), acupotomy group B (stiffness stage intervention only), acupotomy group C (pain and stiffness stages joint intervention). Each group has 8 animals. Except the normal group, other groups were made to be the wind-cold-dampness scapulohumeral periarthritis model by applying the method of mechanical strain plus icecompress method. Except the model group, other groups were treated with acupotomy or electroacupuncture respectively. Acupotomy group A was treated with acupotomy twice in pain period, acupotomy group B twice in stiffness period, and acupotomy group C fourth in pain and stiffness periods. The electroacupuncture group received electroacupuncture treatment every other day, the normal and model groups had non-intervention. After treatment take the plasma, biceps, supraspinatus and infraspinatus muscles to measure the concentration of 5-Hydroxytryptamine, Interleukin-1β, Interleukin-10 and Prostaglandin-E2 in them. By observing variation of above-mentioned inflammatory factors to reveale the effect-acting mechanism of treating scapulohumeral periarthritis in different periods by acupotomy.Results1. The determination results of 5-HT concentration in serum, supraspinatus, infraspinatus and biceps shows:The model group is extremely significantly different compared with blank group. The 5-HT concentration in electroacupuncture group, acupotomy group A and acupotomy group C were statistically obviously lower than model group.2. The determination results of IL-1βconcentration in serum, supraspinatus, infraspinatus and biceps showed:Compared with blank group the model group was statistically significantly different. The IL-1βconcentration in all the treatment groups were statistically sharply lower than that in model group, but all the treatment groups had no statistically significant difference compared with blank group.3. The determination results of IL-10 concentration in serum, supraspinatus, infraspinatus and biceps showed:In the biceps, the concentration of IL-10 in model group was significantly lower compared with blank group。In serum, the supraspinatus and infraspinatus, compared model group to blank, the IL-10 content was significantly increased. In serum compared with model group, the IL-10 content in each treatment group (including cupping group, acupotomology group A, acupotomology group B, acupotomology group C) was obviously lower, and there were extremely significant difference; in supraspinatus the IL-10 content in electroacupuncture group and acupotomy group C decreased obviously compared with model group (P<0.05), these two groups had no significantly difference compared to each other. In infraspinatus, IL-10 content of acupotomology group A reduced significantly than model group and had a significant difference; In biceps, IL-10 content of acupotomology group A group obviously raised compared to model group and hadsignificantly different.4. The determination results of PGE2 concentration in serum, supraspinatus, infraspinatus and biceps showed:Copared model group to the blank, prostaglandin E2 obviously rised, significant difference. In serum each treatment group were significant difference compared with model. In the supraspinatus infraspinatus and biceps acupotomology group C and electroacupuncture group were significantly difference compared with model groupConclusionThrough this research we found that acupotomy loose solution by adjusting the content of inflammatory factor, benignly regulating immune factor, improve systemic and local microcirculation to the purpose of treating scapulohumeral periarthritis. For different period of scapulohumeral periarthritis, acupotomy treatment has the following characteristics:in each period acupotomology treatment can cut harmful inflammatory factor, upward protective factor, and benignly adjusting some immune factors to achieve the purpose of treating scapulohumeral periarthritis. Alleviating the pain and modulating inflammatory reaction is given priority at ache period treatment. Adhesion period treatment is to reduce inflammation primarily. Both two periods treatment is the most effective therapy--not only can alleviate the symptoms, still can better regulation of human body immunity.The effects of both ache period and adhesion period joint intervention and electroacupuncture intervention were better than only intervention in ache period or in adhesion period. There was no significant difference between both two period intervention and electroacupuncture intervention.
