| Objective:to observe the efficacy of acupotome lysis in motional pain points and routine acupuncture in the treatment of acute scapulohumeral periarthritis(wind-cold-dampness type),in order to explore the theoretical mechanism and clinical value of acupotome lysis in motional pain points in the treatment of scapulohumeral periarthritis,and to provide scientific basis for the clinical application of acupotome lysis in motional pain points.Methods:72 patients with acute scapulohumeral periarthritis(wind-cold-dampness type)in the outpatient and inpatient departments of the Department of Acupuncture and moxibustion of Hubei Hospital of traditional Chinese Medicine from March 2018 to December 2019 were selected.the patients were divided into observation group and control group at 1:1.Finally,35 cases were completed in the observation group and 36 cases in the control group.The observation group was treated with acupotome lysis in motional pain points,2 times a week,one week as a course of treatment.The control group was treated with routine acupuncture five times a week and one week as a course of treatment.Both groups were treated for 4 courses and followed up for 6 weeks.In this study,the FPS-R grade distribution,VAS score,Yang Shuxuan shoulder function evaluation score,WHOQOL-BREF score,acupuncture / acupotome safety evaluation and other indexes of the two groups were recorded,and statistical analysis was used to observe the curative effect of the two groups.Results:1.Baseline data: there was no difference in sex,age,course of disease and primary and secondary observation indexes between the two groups(P>0.05).2.FPS-R grade distribution: there was no difference in FPS-R grade distribution between the two groups after treatment and during the follow-up period(P>0.05).There was significant difference in the observation group before and after treatment(P>0.05),while there was significant difference in the control group before and after treatment(P < 0.01).There was no significant difference for two groups in FPS-R grade distribution between the follow-up period and the post-treatment period(P>0.05).During the follow-up period,there was a significant difference for two groups in FPS-R grade distribution between before and after treatment(P<0.01).3.VAS score: there was no significant difference in VAS score between the two groups after treatment and during the follow-up period(P > 0.05).In the comparison before and after treatment,the follow-up period and before treatment,the VAS score was significantly different for two groups(P <0.01).There was no significant difference in VAS score for two groups between the follow-up period and the post-treatment period(P>0.05).4.Yang Shuxuan shoulder function evaluation score: there were significant differences in the distribution of shoulder function and the total score of shoulder function activity between the two groups after treatment(P<0.01),but there was no difference in the scores of internal rotation and external rotation(P>0.05).There was significant difference in the activity scores of touching the ear and the back of the affected hand(P<0.05),and there was significant difference in the distribution of shoulder function during the follow-up period(P<0.05).There was significant difference in the total score of range of motion of shoulder function(P<0.01),but there was no difference in the score of range of motion of shoulder joint(P>0.05).There were significant differences for two groups in shoulder function rating distribution,total score of shoulder range of motion and each score of shoulder range of motion between before and after treatment,follow-up period and pre-treatment(P<0.01).During the follow-up period and after treatment,there was significant difference for two groups in the total score of shoulder range of motion(P<0.05),and there was no difference for two groups in each score of shoulder range of motion(P > 0.05),but there was significant difference in the distribution of shoulder function in control group(P<0.01),and there was no significant difference in the distribution of shoulder function in observation group(P>0.05).5.WHOQOL-BREF score: there were significant differences for two groups in the comparison before and after treatment,the follow-up period and before treatment(P<0.01),but there was no difference for two groups between the follow-up period and after treatment(P>0.05).There was no difference between the two groups after treatment and during the follow-up period(P>0.05).Conclusion:1.Both acupotome lysis in motional pain points therapy and routine acupuncture therapy have a good effect on scapulohumeral periarthritis.2.Both the observation group and the control group had good effects on the improvement of VAS score and FPS-R score,but the control group had more significant improvement in FPS-R score,and the long-term improvement of VAS score and FPS-R score were maintained in both groups.3.Both the observation group and the control group had significant effects on the improvement of shoulder function rating and the total score of shoulder function activity,but the observation group was better in the improvement of total score of shoulder function activity,and the long-term improvement of shoulder function rating and total score of shoulder function activity was better in both groups,while the long-term improvement of shoulder function rating in the control group was better.4.The observation group and the control group had a significant effect on the improvement of shoulder function,but the observation group was better than the control group in the improvement of the affected hand touching the ear and the affected hand touching the back,and the long-term improvement of the two groups was maintained.5.There was significant improvement in WHOQOL-BREF in both the observation group and the control group,and the long-term improvement was maintained. |
