Survey On Health Requirements Of Abdominal Operation

Objective:The survey on contents,patterns,health questions of patients of abdominal operation,aims to offerfing theoretical basis to conduct personal,integrated and perfect health education mode to them.Methods:General condition,health requirements during preoperation,post-operation and leaving hospital,directive patterns leaving hospital and acquirement health information of 71 patients of abdominal operation were investigated in Nanfang hospital.Effect of age,medical spending,culture degree on health requirements and their modes,list of health questions post-operation,difference among 6 pain levels of post-operative health questions,and effection of health questions on laborer groups, were analysed.In the light of bibligraphy,consultation psychologists,statistician and nursing specialists,and preinvestigative result,author planed and revised questionnair oneself.The questionnair include 6 sections,general condition,abdominally preoperative health requirements,abdominally post-operative health requirements, directive patterns loved leaving hospital,means acquiring health information and post-operative complications.Thereinto,second section include 11 items,third section include 4 items.Each item had 5 grades,need very much,need,indifferent, no need very much,no need,and had single choice.Forth section included 9 items, fifth section had 8 items,and them had many choices.Sixth had 19 items,referring relative pain scale,had single choice.Ten investigators invided 4 groups,introduced contents and requairments of questionair,investigate patients and their families of abdominal operation.The investigators conversed with being not families accompanying and cultural patients,recorded investigative results and return questionairs.Statistics means in SPSS13.0 software include frequencies,descriptives and nonparemetric test(2 independent samples tests and K independent samples test).Results:Health requirement level of abdominal operative patients were high very much. Preoperation,patients attached more importance to informations about operative success.Post-operation,health requirement level of abdominal operative patients about being noted if operative success arid service attitude of nurses and docters. Patients hope to be directed by nurses.However,contrast to charge nurse,patients hope to most know the name.the title of a technical post,skill level of charge doctor(P=0.005),and to least know those of anesthetist(contrast to doctor,P=0.000, compare with nurse,P=0.048).Deviation of two preoperation health requairments in four age groups,preoperation,nurse directs patient and patient know importance of preoperative examinations,were significant(P＜0.05).Based on mean rank,the more age,the more requirements.Deviation of two preoperative health requirements in 2 medical spending groups,patients know importance of preoperative preparation, operative methods and procedure,and that of one post-operative health requirement, nurse directing post-operative health questions of patients,were significant(P＜0.05). In the light of mean rank,over three health requirements of patients at one' s own expenses were more significant.Deviation of six preoperative health requirements in three cultural level groups,patients hoping to know the name,the title of a technical post,skill level of charge doctor and nurse,to know regulation in hospital,to know importance of preoperative examinations and preparation,to know operative methods and procedure,and that of one post-operative health requirement,nurse directing post-operative health questions of patients,were significant(P＜0.05).Founded on mean rank,the least culture level,the more health question requirements.The likest direction out hospital of patients was directly conducted by doctor.The telephone was arranged second.Being a lovest means of aquiring health informatios by patients was introduced by nurse.Thus it can be seen,patients were more fonder of communication with nurse face to face.Contrast a lovest means of aquiring health informatios of patients being introduced by nurse to the likest direction out hospital of patients being directly conducted by doctor,deviation was not significant(P＞0.05). Deviation of two directive methods leaving hospital loved by patients in four age groups,telephoning consultation and nurse mailing health handbook to patients,were significant(P＜0.05).Grounded on mean rank,patients of 30 to 49 years required over two means most.Deviation of directive methods leaving hospital loved by patients and acquiring health informations,only lecture about abdominal operation, was significant(P＜0.05).Be based upon mean rank,patients at public expense were the lover over those.Deviation of two methods acquiring health informations in three cultural level groups,compiled handbook by nurse and lectures about abdominal operation,were significant(P＜0.05).Grounded on mean rank,the higher cultural level,the more those health requirements.Pain was primary health question influencing abdominally post-operative patients.Secondly,post-operative health question was PPC.Deviation of single post-operative health questions in six pain grade groups,efficacious spiting out phlegm(rate of occuring to wound pain in effecting factor was 22.5%),was significant(P＜0.05).Based on mean rank,the more increasing pain level,the more coming forth not efficacious spiting out phlegm. Deviation of two post-operative health questions in six pain grade groups,patients hoping to know regulation in hospital and to know the importance of preoperative examination,was significant(P＜0.05).Deviation of two post-operative health questions in two laborer groups,PPC and self care ability,was significant(P＜0.05). Founded on mean rank,those questions in laborer group of physical force were more significant.Conclusions:Health requirement level were high very much of abdominally operative patients.Patients recognize both of doctor and nurse can be educator.Being lovest methods of health education were communication and lectures because of those communicating patients with nurse or doctor face to face.Besides,conveniently telephoning consultation was fonded too.Health education handbooks compiled by nurse or doctor were fonded by middle age,young and high-culture patients.Nurse should evaluate enough age,medical cost,cultural level,etc,of abdominally operative patients before health education,conduct personal education,and attach importence to study teaching methods such as web,CD-ROOM,video etc.Nurse would explore personal,full-course,excellent health education model for them,so that satisfing their kinds of health requirements.Nurse should collaborate with anesthesist to alleviate pain of patients,direct abdominally post-operative body posture and post-operative complications,strengthen education about hospital regulation and importance of preoperative examinations.Nurse analysed enough causes of occurring health questions before education,so that have a definite object in view to solve them.
