Objectives: we measured the level of prostacyclin, ThromboxaneA2,platelet adhesion and aggregation,activation of FactorⅡ,activation of Factor Ⅶ,D-dimer in the plasma before and after operation in order to acquire why the patients with pelvic and malignant operation prone to lower deep venous thrombosis(DVT)? Which indexes of these can monitored the level of Hypercoagulable state and have important function to prevent lower deep venous thrombosis. Prostacyclin(PGI2) and ThromboxaneA2 have important regulation to Platelet aggregation ,adjust of vascular tension and thrombosis ,also have complicate regulation to the growth ,differentiation and metastasis of carcinoma cell. Operation and trauma make platelet activation because of the wounded vascular .There is no relationship between lower deep venous thrombosis and the amount of platelet ,but platelet activation is related with it. In the patients of carcinoma the platelet is activated before operation . The carcinoma cell and activated platelet make blood in Hypercoagulable state(HCS) by starting coagulation mechanism. The activation and antigen content ofcoagulation Factor are important to thrombosis. It is recently proved that D-dimer is very sensitive to Hypercogulable state and thrombus diseases .we measured the level of PGI2 ,TXA2, FⅡ:C, FⅦ:C,rate of platelet adhesion and aggregation ,the content of D-dimer in the plasma of abdominal and pelvic patients (25 malignant patients in it) before operation and after operation 1st,3rd,5th day and studied the effect of these changes significant difference between these operations . Methods: PGI2 , TXA2, Factor Ⅱ, Ⅶ, D-dimer rate of platelet adhesion and aggregation were measured from 56 patients between malignant patients and no malignant patients with abdominal and pelvic operations.The patients were divided into four groups , no malignant group from abdominal patients, no malignant group from pelvic patients ,malignant group from abdominal patients, malignant group from pelvic patients.(The effects was by this order) Results: 1.The rate of platelet adhesion was increased after operation (before operation ,25.38±8.10 ,23.17±9.24 28.23±7.14 ,30.30±8.25,) but it is the highest from in the 3rd day after operation (33.01±9.11 32.23±8.08 30.19±8.27 36.17±10.99 ) p<0.05.The level in pelvic malignant group was maintained in the 7th day (to the level of 3rd day).The rate of platelet aggregation was resemble with the adhesion.2.The changes of PGI2 is higher than before operation (92.26±65.35 135.15±109.20 151.21±114.15 149.04±109.20 P>0.05 ), but after operation ,it was kept going down. Lowest in the 7th day after operation(40.21±15.17 46.35±16.75 80.77±31.56 91.77±30.23 P<0.001).It had significant change between malignant group and abdominal group (no malignant),P<0.05.3.The changes of TXB2 was increased after operation every group 33.37±19.28 85.27±61.75 82.89±53.34 90.17±60.15 p>0.05 ,but it was significant between malignant pelvic group and abdominal group (no malignant),p<0.001.It was the most high in every group in 3rd day and 2--5 times than before operation and descended after 3rd day.4.FⅡ:C was increased after operation ,highest in the 3rd day (76.31±41.51 120.25±72.98 172.34±180.27 174.29±179.23) It was significant with before operation, p<0.05,and no significant between abdominal and pelvic group p>0.05,it descended in every group from 3rd day to 7th day(60.17±34.15 108.19±67.24 150.21±81.21 132.17±79.63 p<0.05 . FⅦ:C was resemble with FⅡ:C.5.It was significant among every group , p<0.05.The D-dimer positive ratio was higher in malignant group than in no malignant group . Conclusion: 1.Due to the effects to the vascular endothelial cell (PGI2),platelet function (adhesion and aggregation, TXA2), blood coagulation (FactorⅡ Ⅶ), fibrinolysis (D-dimer) from operation ,patients after operation was in HCS ,it was the highest from 1st to 3rd day .2.But in malignant and pelvic...
