Clinical Study On Time-dependent Analgesic Effect Of Treatment Of Lumbar Disc Herniation By Combining Acupuncture At Jiaji Acupoint With Fu's Subcutaneous Needling

Objective:To explore the synergy to treat LDH between acupuncture Jiaji acupoint and Fu's subcutaneous needling and supply an acupuncture comprehensive scheme which has complementary advantages and synergistic effect, by observing this method's analgesic effect acting speed, the effect-lasting time, the instant and recent analgesic effect and the comprehensive therapeutic effects.Methods:60 patients in total with LDH were selected to be observed. They were divided randomly into the treatment group (n=30) and the control group (n=30). The treatment group selected acupuncture Jiaji acupoint combing with Fu's subcutaneous needling:Firstly, Fu's subcutaneous needling was applied. Depend on the theory of Fu's subcutaneous needling and the different pain areas, the myofascial trigger points were made clear and definite. In the Fu's subcutaneous needling,the needle was inserted horizontally beneath the skin, accurately, and then fanned to and fro.The needle was manipulated for about 2-3 minutes and was then retained for 30 minutes,with fanning manipulation applied at every 10 minutes interval. The needle was not taken off until the pain was completely disappeared, but the plastic tube was still left in the body and fixed it with infusion stickers for at least 6 hours. Secondly, acupuncture Jiaji of both sides corresponding to the protrusion, and Huantiao(GB 30) on the affected side of protrusion combined with Yanglingquan (GB 34) for L4-5 disc protrusion or Weizhong (BL 40) for L5-S1 disc protrusion. After the electric-shock like needling sensation was conducted downward to the lower limb, the needle was remained for 30 minutes.The control group selected Fu's subcutaneous needling was simply applied. Both groups were treated every day, five days in succession with 2 days interval in between,10 times constituting a treating course. Observation was conducted over 2 courses. The acting time of analgesic effect and the effect-lasting time after the first treatment were observed. The pain was assessed comprehensively by using the internationally recognized McGill Pain Questionnaire which describs and measures pain. Comparision were made between the two groups on the mean pain score before treatment, after the first treatment (instant effect) and after two treatment course (recent effect). The therapeutic effects were measured by using Yang's quantitative evaluation scale on the symptoms and complaints,the ability of work and living,the lower limbs' function and the clinical signs. The comprehensive therapeutic effects were assessed depend on the state administration of traditional Chinese medicine. All data were handled by software SPSS11.5. results:(1)There was no obvious statistical difference on the acting time of analgesic effect between the two groups, but there was significant statistical difference on the effect-lasting time. The effect-lasting time of the treatment group was longer than the control group.(2)Compared with the scores of of PRI,VAS,PPI before treatment measured by McGill Pain Questionnaire, the scores decreased significantly after the first treatment and 2 treating courses(P<0.01);comparision between the two groups, there was no obvious difference on these three scores after the first treatment (P>0.05),but there was significant statistical difference after 2 treating courses (P<0.01). The mean pain score of the treatment group is lower than the control group.(3)After 2 courses of treatment, there showed significant statistical difference between the two groups (P<0.01), on the score of therapeutic effects and the excellence rate. The treatment group showed higher therapeutic effects, higher excellent rate and higher recovery rate than the control group.(4) After 2 courses of treatment, results is indicated that both two groups'efficiency ratio reaches 100%. But the curative ratio of the treatment group is higher than the control group (P <0.01)Conclusion:Both therapies are effective. Comparisons between the two groups have shown that acupuncture Jiaji point, combinined with Fu's subcutaneous needling is better than the Fu's subcutaneous needling simply group on the effect-lasting time, the recent analgesic effect and the comprehensive therapeutic effects.
