Research Of Patient Compliance And Its Influence Factor After Coronary Stent Implantation

Objectives:The purpose of this research is to explore the level of patient compliance, social support and coping style in patient after coronary stent implantation, to analyze the relationship between them, to further adopt the measure to enhance the treatment compliance, and to improve the second-level prevention of patient with coronary heart disease.Methods:This research uses random sampling method, uses the telephone revisit to 257 patients after coronary stent implantation. The tools of this investigation including generally material questionnaire, the treatment compliance questionnaire, the society supports questionnaire, and the medical coping modes quationnaire. This research uses the SPSS 17.0 statistics software to carry on the descriptive analysis, the Chi-squre examination, the T-test, the single factor variance analysis examination, and the Pearson's correlation analysis to the materials.Results:1. The level of the three aspects of treatment compliance of the patients after coronary stent implantation was different. The compliance of taking medicine of 145 patients was good, accounts for 56.42%. In the range of reexamination compliance, the outpatient service reexamination was the best, accounts for 77.43%; the coronary radiography reexamining compliance was the worst, accounts for 14.01%. In the life style compliance,91.05%patients were good to the food habit compliance.2. In different standard of culture and medical expense payment pattern, the compliance of taking medicine had statistical difference (p<0.01). In different income and number of implanted stents, the compliance of taking medicine had statistical difference (p<0.05). In different standard of culture,age and medical expense payment pattern, the compliance of outpatient service reexamination had statistical difference (p<0.01). In different standard of culture, the compliance of life style had statistical difference (p<0.01).3. The score of patient's society supports were in the medium level. In different occupation and standard of culture, the total score of patient's society supports had statistical difference (p<0.01). In different marital status, the total score of patients society supports had statistical difference (p<0.05). In different marital status, standard of culture and medical expense payment pattern, the score of patient's objective and subjective support had statistical difference (p<0.01). In different gender and occupation, the score of patient's support exploitability had statistical difference (p<0.05).4. There was positive correlation between the objective support and the reexamination coronary radiography (r=0.12) and the movement compliance (r=0.13) (p<0.05). There was significantly positive correlation between subjective support and the movement (r=0.14) and the diet(r=0.15) (p<0.05). There was significantly positive correlation between support exploitability and banning smoking (r=0.23) (p<0.05).5. After coronary stent implantation, the patient tended to adopt facing coping style. There was positive correlation between facing coping style and blood sugar, blood pressure and blood fats control situation (r=0.14) and outpatient service reexamination (r=0.14). There was negative correlation between evasion coping style and the compliance of taking medicine (r=-0.15), reexamination of treadmill exercise testm (r=-0.14)and limiting the liquor (r=-0.14). There was negative correlation between submitting coping style and banning smoking (r=-0.13) and movement (r=-0.14).Conclusion: 1. This survey result showed that after coronary stent implantation, patient's treatment compliance disposition to high. Patient's standard of culture, the month income and the medical expense payment pattern, the inhabited area can affect the treatment compliance of patients. The hazard factor's control and the coronary radiography reexamination's compliance needs further improving.2. The patient after coronary stent implantation had earned the high social support, after being out of hospital, patient preferred choosing facing coping style to evasion and submitting coping style.3.There was positive correlation between the patient's treatment compliance overall and the society support three dimension, which means the patient who got higher society supports scores tended to treat the compliance to be better. In turn, the treatment compliance good patient would also seek and uses various aspects the social support, further enhancing the treatment compliance.4. The more patient tend to adopt facing coping style, better treatment compliance. The more patient tend to adopt evasion and submitting coping style, worse treatment compliance.
