Telemedicine have important significance to raise our people's living standards. Telemedicine include Remote Diagnosis, Consultation of Specialists, Information Service, Online Examination, Remote Studing,etc.Remote high-quality medical help to enhance the people's health, particularly in the elderly and people with chronic diseases care has advantages; It also has the potential to engage in inter-regional consultation of difficult cases, to improve medical quality effectively in some areas. Remote Medical Consultation between patients and medical experts set up new contacts so that the patients in home or in local hospital can be diagnosed and be treated under the guidance of experts faraway. Doctors and patients thus saving a lot of time and money.Modern technology is progressing rapidly in communication, computer and image processing.various international organizations and groups developped a series of high-performance multimedia algorithms and standards; Network Technology was rapidly popularized. Telemedicine had a certain amount of technology development space, and remote diagnosis becomes more feasible.Telemedicine more focus on medical information, such as various medical imaging, patient files and medical information. DICOM is the fact international standards of the medical digital imaging and communications, So it is need to develop middleware to achieve complete medical information in the process of the integration of hospital information system software and video conferencing.This thesis described a blue print of a remote and real-time medical systemby comparing and applying various new technologies. The system providesthe level of QoS and security from end to end. . The network is not only carrying recent technology,the entire network MPLS, rapid convergence and FRR, but also QoS technology based on Diffrentiated Services (DiffServ) mode, dividing various business classes, different bandwidth allocation for different grades, and using different queue scheduling mechanism, resolve QoS of the IP backbone network. ADSL network are suitable for using DiffServ framework. ADSL differentiated services need to be expanded, high capacity and intelligent DSLAM , which provides IP transfer functions , support for the DiffServ QoS mechanisms and user management functions.
