In recent years, Chinese medical and health service has made considerable progress. All levels of hospitals, community health service institutions and professional health organizations have established a dedicated hospital information system and improved work efficiency. However, the hospitals still unable to share information with each other owning to the limited to management and system design. When referral patients from community hospital to a higher-level hospital, the diagnosis in community hospital and health information can not be transferred simultaneously. This forms information islands in various medical institutions. To achieve data sharing among hospitals, which is the demands of people, and is also the development trend of hospital information system.Several years ago, the foreign countries brought up the concept of Integrating Healthcare Enterprise (IHE) to study how to make good use of HL7, DICOM and other existing standards and to build a regional medical information network. Based on the HL7, combine with the needs of establishing hospital information sharing system, this paper gives us a design and implementation of tow-way referral system. The main work of this paper as follows:1. Make a research in data transfer and data sharing support technology, put forth the model which use HL7 as the standard ,message-based middleware in the hospital two-way referral system;2. Combine with the actual business of the patient referral and based on the system detailed analysis, this paper offers a detailed design of the two-way referral system, including functional design, business process flow design, database design, etc.;3. Using PowerBuilder tool to achieve the hospital referral system in Windows environment. This paper offers the main functions composition of each module, associated code definitions and operation instructions.4. Using MOM Message Oriented Middleware and J2EE tools to implement HL7-based message exchange module in Windows, and Oracle Solaris environment. Given the composition of the main functional modules, code definitions and the message processing instructions by this paper.In this paper, the design and implementation use UML method. After the evaluating and trial, the system basically reaches the data sharing among the hospitals and complies the special requirements. It sets a good example to the development of similar systems.
