Objective1. To explore the the principle and method of diagnosing and treating towards the "Fluid Retention Diseases" and theory origin of "The spleen is the source of production of sputum, lung sputum for storage".2. To explore the mechanism of improvement of mucus hypersecretion in COPD rats by Jian Pi Yi Fei Hua Tan Decoction.Method1. Put the "Fluid Retention Diseases" in "Jin Gui Yao Lue" by Zhang Zhong-jing as the core, On the basis of fluid metabolism related theory in "HuangDiNeiJing", and then analysis from the perspective of principle, method, prescription and medicine.2. The rat model of COPD was established by tobacco smoke inhalation, and then the rats were randomly divided into six groups including the normal group,model group, jianpiyifei Decoction group,huatan Decoction group jianpiyifeihuatan Decoction group and Carbocisteine group. The normal group and model group were given normal saline solution intragastrically and other groups were given relevant medicines for 28 days, respectively. The pulmonary function tests and arterial blood gas analysis were taken to find the lung ventilation and the blood flow changes. The expressions of IL-13, phospho-STAT6 and MUC5AC mRNA in lung were detected by ELISA, western blot or RT-PCR, respectively. Spectrophotometry to detect the contents of serum D-lactic acid and DAO, radiation immunity analysis method to detect the serum GAS,MTL and SS, CCK,VIP,SP, phenol red expectoration drainage experiment to determine airway amount of phenol red expectoration drainage in rats.ResultLung function and Aterial blood gas analysis:compared with the normal group,the relevant indicators were abnormal conditions in model group:the level of FEV0.3/FVC、pH、 PaO2、SaO2 were decreased,the PaCO2 was increased; compared with the model group, the above revant indicators had been improved including Jianpiyifei group,huatan group jianpiyifeihuatan group,especially the Jianpiyifeihuatan group.The expressions of IL-13, phospho-STAT6 and MUC5ACmRNA in lung: compared with normal group, the expressions of IL-13, phospho-STAT6 and MUC5ACmRNA in model group rats were significantly higher (P<0.01). Compared with model group, the expressions of IL-13, phospho-STAT6 and MUC5ACmRNA in all treating group rats were significantly lower (P<0.01)Gastrointestinal hormones change:compared with normal group, model group rats serum GAS, MTL and SS, CCK and SP content were lower (P<0.05 or P<0.01), the VIP content increased significantly (P<0.01). Compared with model group, the dosage groups of rats serum GAS, MTL and SS, CCK and SP content were increased (P<0.05), the VIP content was reduced (P<0.01); Comparison between the three Chinese medicine treating groups, there was no significant statistically difference, but from the numerical point of view, Jianpiyifei Decoction group, Jianpiyifeihuatan Decoction group serum GAS, MTL and SS, CCK and SP content has a tendency to rise further and the VIP content further reduce.Gastrointestinal mucous membrane barrier function change:compared with normal group, model group rats serum D-lactic acid and DAO levels were significantly higher (P<0.05), suggesting that the COPD model rats gastrointestinal mucous membrane barrier functionwas impaired. Compared with model group, the dosage groups of rats serum D-lactic acid and DAO levels were significantly lower (.P<0.05); Comparison between the three groups of Chinese medicine treating groups, there was no statistically significant difference, but from the numerical point of view, the Jianpiyifeihuatan group rats further reduce the level of serum D-lactic acid and DAO. Prompting that Jianpiyifeihuatan Decoction can repair and improve the intestinal mucosal barrier function.Phenol red expectoration drainage experiment results:compared with the normal group, the phenol red expectoration drainage volume in airway of model group rats were significantly higher (P<0.05),compared with model group, the airway row of phlegm quantity in all treating group rats were significantly lower (P<0.01),comparison between three Chinese medicine treating groups, the airway row of phlegm quantity in Jianpiyifeihuatan Decoction group is the lowest, the most close to the normal group.Conclusion1. Zheng qi is the fundamental of Phlegm retention disease, it’s pathological mechanism for stomach empty water stops.2. Zhongjing put forward the medicine and the treatment principlesto restore normal operation of the body fluid into the ultimate goal. The mechanism is included in zhongjing " "If the viscera function is normal, the human body is harmonious".3. The treatment of phlegm retention to eliminate pathogenic factors and essence. According to the disease, According to the the occasion, Take sweat or purgative method to give ways for the pathogens. In addition,Zhongjing stressed that long illness may not lead to weak, it still need give priority to eliminate pathogenic.4. JianPi Yi Fei Hua Tan Decoction improves airway mucus hypersecretion in COPD rats through inhibition of "IL-13-pSTAT6-MUC5AC" signaling pathway.5. Jianpiyifeihuatan Decoction can improve the gastrointestinal motor functionand the level of gastrointestinal mucous membrane barrier of the COPD rats, what’s more, it can reduce the airway row of phlegm in COPD rats, this could be one of the mechanisms of the effects of JianPi Yi Fei Hua Tan series Decoction in COPD.
