ObjectiveAnalyzed the sensitive indexes of clinical symptom of digestive tract fistula and the characteristics of abdominal cavity drainage fluid after gastrointestinal surgery, so as to establish clinical observational evaluation index for digestive fistula and conclude the clinical meaning of abdominal cavity drainage fluid.Those are easily mastered by the medical staff and provide the basis for judging the occurrence of complications. It is of great significance in patients' recovery.Methods1.Used the Delphi method to set up observational evaluation indexes of the digestive tract fistula of gastrointestinal complication after the operation.2. Collected abdominal drainage fluid of 268 cases with gastrointestinal surgery. Made and recorded laboratory tests which offer an experimental basis for establishing a reliable standard reference range for abdominal cavity drainage fluid used to detect postoperative complications.Results1. The observational evaluation indexes of gastrointestinal surgery complicated with digestive tract fistula were determined through two rounds of expert consultation. It includes 5 first grade indexes: ?bservation for vital signs, chills, and mental health ? observation for patient's abdominal symptoms and signs ? observation for abdominal drainage fluid.?the patient's general situation.? laboratory results and auxiliary examination and 25 second grade indexes.2. The observation of abdominal drainage fluid was the key index for gastrointestinal surgery complicated with digestive tract fistula. The experts reached an agreement:The first grade index:C. abdominal drainage fluid observation, the importance of assignment 4.85±0.07 full marks 0.90.The second grade index:C1 abdominal drainage fluid appearance (color, coagulability and transparency), importance of assignment 4.95±0.22 full marks0.95.3. Monitoring of abdominal drainage fluid color provides reference for judging the occurrence of postoperative complications. The study found on the third day after gastrointestinal surgery abdominal drainage fluid color are dark red 200 cases(76.3%), reddish brown33cases(12.6%), reddish 6 cases(2.2%), flaxen 23 cases (8.8%), if there are any other colors, we should pay attention.4. The analysis of the characteristics of abdominal drainage fluid after gastrointestinal surgery (268 cases) provides clinical reference basis.The laboratory test indexs (color, transparency, density, PH value, the total number of cells, white blood cells, multinucleated cells number, the total number of monocytes, sugar, protein, and chloride content) provide data resources for related research.5. This study found that the abdominal drainage fluid contains a certain amount of protein after gastrointestinal surgery. This study found the protein loss of drainage fluid is 39 g/L.Conclusion1. Mastering observational evaluation indexes for gastrointestinal surgery complicated with digestive tract fistula could improve nurse observational ability for complications and improve the safety of patients.2. The study described the characteristics and clinical significance of abdominal cavity drainage fluid for patients after gastrointestinal surgery, and provided an experimental basis for establishing a reliable standard reference range for abdominal cavity drainage fluid used to screen for postoperative complications.
