Objective:To Observe the intervention of Pingfeng Qinglong Decoction on inflamematory indexes in AECOPD(asthenic lung Phlegm and retained fluid)treatment,compare the changes of inflammatory indexes during the hospitalization of two groups of AECOPD patients,and analyze with reasonable statistical analysis methods to verify the effectiveness and safety of Pingfeng Qinglong Decoction in the treatment of AECOPD to provide ideas for Chinese medicine in the treatment of AECOPD.Methods:72 Patients were selected from the lung disease department of Yunnan Provincial HosPital of traditional Chinese Medicine(Guanghua and Dianchi department).They were randomly divided into a treatment group and a control group with 36 cases in each group,who were diagnosed as AECOPD(asthenic lung Phlegm and retained fluid).Patients in the control group were treated with cefodizime sodium for injection,ambroxol hydrochloride for injection,budesonide for inhalation,ipratropium bromide solution suspension for inhalation,and basic treatment,patients in the treatment group were given Pingfeng Qinglong Decoction on the basis of treatment in the control group.The treatment course of the two groups was 10 days.After the treatment course,compared the two groups before and after treatment of the TCM symptoms,pulmonary function indexes and inflammator.Results:One patient of the treatment group was rejected couse of refused to take Chinese medicine.One patient in the control group deteriorated and transferred to ICU without completing treatment.There were 35 patients who completed the treatment in the treatment group and the control group.After 10 days of treatment,the results are as follows:1.Comparison of comprehensive efficacy:The total effective rate of treatment in the treatment group was 85.71%,which was higher than 77.14%in the control group.The therapeutic group was significantly better than the control group in terms of overall efficacy.2.Comparison of total scores of TCM syndromes:After treatment,the total scores of syndromes in both groups decreased significantly.After treatment,the total score of the treatment group was more significant than that of the control group,indicating that the treatment group was more helpful to improve the TCM syndromes of patients.3.Single symptom score comparison:After treatment,the symptom scores of the main symptoms(cough,sputum,shortness of breath,fatigue and sweating)of the two groups of patients were lower than before treatment,and the difference was statistically significant(P<0.05);Comparison of secondary symptom scores(shortness of breath and chest tightness,less diet and loose stool)between the two groups of patients before and after treatment,there was no difference in contrast improvement between the control group before and after treatment of less diet and loose stool in the control group(P>0.05),others symptoms improved in both groups(P<0.05).Compared with the control group,the improvement of the symptom scores of the primary and secondary symptoms in the treatment group was significant(P<0.05),which was statistically significant.4.Intervention results of inflammatory indicators:The inflammatory indicators(WBC,hs-CRP,SAA,PCT)were improved to varying degrees after treatment and intervention.The inflammatory indicators(WBC,hs-CRP,SAA,PCT)of the treatment group were improved after treatment)Compared with the control group,P<0.05).The treatment group is batter than the control group in the inflammatory indicators(WBC,hs-CRP,SAA,PCT).5.Comparison of blood gas analysis:The blood gas analysis results(PaO2,PaCO2)of the two groups of patients were improved after treatment.After treatment,the PaO2 and PaCO2 values of the treatment group were more significantly improved than the control group,which was statistically significant.The treatment group had an advantage in improving blood gas results.6.Pulmonary function comparison:FEV1/FVC%and FEV1%of the two groups after treatment were higher than those before treatment,and both groups were effective.The improvement of the FEV1/FVC%and FEV1%values in the treatment group after treatment was significantly different from that in the control group(P=0.004<0.05),which was statistically significant.Therefore,the improvement of lung function in the treatment group is better than that in the control group.7.Evaluation of safety indicators:Heart rate,blood pressure,respiration,liver function,kidney function and other indicators of the two groups of patients were within the normal range before and after treatment.There were safety and reliable during the treatment of the two groups of patients.Conclusion:On the basis of western medicine treatment,using Pingfeng Qinglong Decoction to treat patients with AECOPD(asthenic lung Phlegm and retained fluid),the treatment group is better than the control group in improving symptoms,lung function,inflammatory indicators,and arterial blood gas analysis.Prove the advantages of traditional Chinese and western medicine in the treatment of AECOPD. |
