Objective:To determine correlation between syndrome and dermoscopic images patterns of patients with AGA.Thereby futher explore evoluting regulation between blood heat syndrome and deficiency of liver and kidney syndrome of AGA.Methods:Total 15 AGA patients with blood heat syndrome and 15 patients with deficiency of liver and kidneysyndrome, took 45 photographs each group.Observe the differences of dermoscopic images between two syndromes above on the basis of observation target as Peripilar Signs(PPS)、Yellow Dots、Blood vessels Signs.Results:Amounts of PPS:deficiency of liver and kidney syndrome(22.8±3.73)is higher than blood heat syndrome (5.83±4.513).There were statisticall differences between two syndromes above (P=0.009<0.05); positive rate of blood vessels on bilateral frontalangles and hair vortex of vertex:deficiency of liver and kidneysyndrome is lower than blood heat syndrome, the differences between two syndromes above were statistically significant by chi-square test; positive rate of Yellow Dots on bilateral frontalangles and hair vortex of vertex:deficiency of liver and kidney syndrome is lower than blood heat syndrome, there was no statisticall differences between two syndromes above; positive rate of Honeycomb Signs on bilateral frontalangles and hair vortex of vertex:deficiency of liver and kidney syndrome is lower than blood heat syndrome,there was no statistical difference between two syndromes above.Conclusion:There were differences between dermoscopic digital images of different syndromes of AGA. Our research would provide experimental basis for application of dermoscopy in diagnosis and treatment of AGA by Traditional Chinese Medicine.Significance:AGA is a common disease in clinic which could be diagnosed by past history and skin lesion.Dermoscopy has value in early and differential diagnosis of AGA、previous studies have demonstrated that dermoscopic signs of AGA includes Peripilar Signs、Yellow Dots、Blood vessels Signs、Honeycomb Signs.The Peripilar Signs correspond to inflammatory infiltration around hair follicle in pathological examination, which could be observed in early stage of AGA as a brown depressed halo at the follicular opening. Yellow Dots Signs manifest as multi-annular tissues which can be seen in advanced cases, symbolize a consequence of sebum and keratin accumulation within dilated follicular infundibula. Blood vessels Signs refer to dilated capillaries which can be seen as erythema, dots、globules and linear vessels under dermoscopic vision. Honeycomb Signs refer to diffuse honeycomb-type hyperpigmentation on scalp. In the point of TCM, blood heat and yin deficiency are the most important pathogenesis of seborrheic alopecia.Multi-factors cause yin deficiency or excessive heat in the blood system and result in hair lossing of moistening.To some extent, yin deficiency syndrome can transforms into blood heat syndrome. In this study,we selecte patients of AGA in blood heat or yin deficiency syndrome based on the methods of syndrome differentiation above and compare the differences of dermoscopic images between two syndromes baesd on observation indexes of Peripilar Signs、Bloodvessels、Yellow Dots、Signs、 Honeycomb Signs. We take advantage of the demoscopy to study the dermoscopic images of seborrheic alopecia patients in different syndromes. The results show that difference exists in different syndrome’s dermoscopy image.The demoscopy has certain value in TCM differentiation and treatment in seborrheic alopecia.The image may play a supporting role in the clinical dermatology of TCM.
