Study On Experience Data Mining Of Prescription Of Rheumatoid Arthritis In Active Stage

Obgective:By collecting tutor’s prescription medications in the help of datating mining, summarize associate professor he houfu’s experience and theoretical thought in clinical diagnosis and treatment of active rheumatoid arthritis on the basis of his clinic practice for almost thirty years, in order to have further improve clinical efficacy in the treatment of rheumatoid arthritis by traditional Chinese medicine.Methods:Collect tutor’s all the cases of active rheumatoid arthritis in nearly t-hree years. Record all the prescription herbs which were picked up according to the inclusion criteria and exclusion criteria.Establish a database with Excel 2007, and then entry the data of prescription herbs with the help of Eptidata 3.1.Data mining technology in Spss21.0 like frequency statistics, cross analysis and hierarchical cluster analysis were used to find out the high-frequency herbs cl-asses and commonly used couple herbs as well as the close relationship betwe-en cluster consisting of herbs.On this basis to discuss tutor’s experience of fu-ndamental prescriptions and theoretical thought in clinical diagnosis and treatment of rheumatoid arthritis.Results:53 cases were sreened from all the cases which were include 46 females and 7 males with an average age of 58.48 years old and male to female ratio was about 1:6.7. Prescription of 53 pieces were analyzed, and the average dosage of single Prescription was 15.8,about 83% Prescription dosage blow 16. Doing frequency analysis in the sreened 53 prescriptions received 71 kinds of herbs whose total frequency was 837 times, with using frequency in more than 25 times of 15 kinds, like radix gentianae macrophyllae,Chinese starjasmine, notopterygium, zedoary, sappanwood, yanhusuo, ramulus mori, orientvine, glandularstalk, pinus -tabulaeformis, scorpion, pepper stem, lumbricus, paniculate swallowwort root,radix puerariae,which were the most commonly used herbs.The 39 kinds of herbs which frequency was above 3 times were the most commonly appeared in the prescriptions.There were 11 types of herb involved in common Prescriptions, the top six were antirheumatic Chinese medicinal herbs, herbs for inviqorating blood circulation and eliminating stasis, clean damp and diaphoretic recipes Chinese medicinal herbs, inducing diuresie for removing edema Chinese herbs, search choroid Chinese herbs, heat-cleaning and inviqorating blood circulation Chinese herbs. The hierarchical cluster analysis was performed on the frequency of use of 3times above 39 kinds of herbs.The analysis shows that there are 5 clustering prescriptions and 10 groups of commonly used couple herbs including the two fundamental prescriptions and three plus and minus medicine groups,and commonly used aggregation for internal medincine diseases.Conclusion:Instructor hold the view that cold,wet,stasis heat are the main pathological factors in active rheumatoid arthritis.Throughout the pathogenesis, dampness and blood stasis are alawys presented in the entire disease process. Symptoms characteristic like alpine stasis syndrome as well as heat stasis syndrome can be manifested in active rheumatoid arthritis,you can also be expressed as mixed cold and heat. We find out that the main effect of the two fundamental prescriptions is eliminating wind and dampness,promoting blood circulation and analgesics, tonifying liver and Kinney.The main effect of the three plus or minus medincine group are liminating heat dampness and cold dampness herb groups and group which is acting on the upper limbs. We can learn that the main treatment of rheumatoid arthritis is liminating wind and dampness,promoting blood circulation and getting rid of stasis,with eliminating cold,removing heat,tonifying kidney,eliminating phlegm.If we try to treat the RA in Chinese medicine,first,we must find out that the disease is on either activity or remission;second, deficiency and excess should be distinguished in human’s body; third,identified the focus of RA is cold or heat,and then identified the pain mainly in the upper or lower extremities;finally,therapy other intermal medincine diseases.The treatment of internal medicine diseases with rheumatoid arthritis should be payed attention to. First,patients with anxiety-depression can be prescribed ganmai-dazao decoction,banxia-Houpu decoction,Xiaochaihu decoction;patients with insomnia can be prescribed Baihe- DiHuang decoction, patients with CHD can be prescribed Gualou-Xiebai decoction,Danshen decoction; patients with stomach discomfort which caused by taking non-steroidal anti-inflammatory drugs or other reason can be prescribed Zuojin-decoction,Xiaochaihu decoction.
