To observe the efficacy of the compound Tutengcaotang on gouty arthritis and experimental gouty renal damage in rats, investigate the Effect of gout on renal damage and the Effect of compound Tutengcaotang for chronic gouty arthritis and uric acid renal damage.Clinical part:Chronic gouty arthritis patients met the diagnostic criteria for a total of 82 patients were randomly divided into treatment group 40 patients and control group 42 patients. Basic treatment was oral allopurinol 0.25g/day. When blood uric acid level control in 300umol/1, adjusted to 0.25g/every other day. If the acute attack of gout was eased, oral administration of Allopurinol Capsules 0.25g/the next day, then increase to 0.25g/day. The treatment group was treated by the compound Tutengcao. The control group was given Tongzhisurunjiang. Before and after the treatment, getting TCM symptom scores, changes in laboratory parameters, and statistical comparative analysis.Results:The total effective rate was 97.5% in the control group, the total effective rate was 90.5%, the two groups before and after treatment were significantly different, between the treatment group and the control group (P<0.05); the treatment group and control group could improve symptoms and signs of joint pain, joint swelling, were significantly improved range of motion (P<0.05), treatment group and control group, the effect is not significant difference (P> 0.05); the treatment group and Chinese medicine can improve symptoms of the control group patients (P<0.01, P<0.05), before and after treatment of symptoms total score improved significantly (P<0.05), while the effect of the treatment group than the control group (P<0.05); the treatment group and in the control group could significantly lower blood uric acid, serum creatinine, urea nitrogen and urine protein levels (P<0.05), and the effect of the treatment group than the control group (P<0.05).Experimental part:Health SPF 105 male SD rats were randomly divided into normal group and model group, where in the normal group 6, model group 99. Semi-automatic biochemical analyzer, in accordance with the relevant kit instructions, for 0,2,4,6,8 week collected 24h urine and serum testing, each rat was measured uPRO/24h, sUA, sCr, BUN values.6,8 weeks were killed in the amount of 1/2 of the rats, remove both kidneys, along the outside edge of the left kidney longitudinal, transverse rear 1/2 kidney tissue, whichever is the lower half quickly put 10%neutral formalin fixed, paraffin embedded sections of tissue sections by conventional methods HE and PAS staining, light microscopic observations.Results:Compound Tutengcaotang can reduce blood levels of uric in rats, reduce blood urea nitrogen and creatinine, improve renal function and reduce proteinuria, reduce renal interstitial purine deposition, reducing urate crystals to renal interstitial, tubular inflammation. Conclusion:Compound Tutengcaotang can effectively improve the treatment of chronic gout in patients with symptoms and signs, to reduce serum uric acid, serum creatinine, urea nitrogen and urine protein levels, protect renal function. Experiment to improve uric acid kidney damage in rats blood uric acid levels, lower blood urea nitrogen and creatinine, improve renal function and reduce proteinuria and protect renal structure. Chinese herbal compound described Vine soil grass soup can be effective in the treatment of chronic gout to reduce uric acid, creatinine, urea nitrogen, albuminuria levels, improve gouty kidney damage.
