Picture archiving and communication system comes from computer technology and medical image technology. Digital imaging and communications in medicine is a international standard about medical image in the medical informatics domain. The manufacturer and user of the medical equipment implement the interconnectivity through the standard network by it. It becomes a standard about medical image, and it is a focus in the world at present.This paper introduces the development and history of DICOM standard at first, and illuminates component of standard briefly. DICOM includes 16 chapters. Every chapter is about a certain criterion in order to be revised and expanded easy, they are not only independent but also interrelated. The structure and content of standard is depicted by the cooperation of all chapters.Object-oriented technology is a mainstream of the development of the software in the future. The Object-oriented technology is applied to our system because it can improve deeply quality and maintenance and re-use of software.DICOM model of the real world is an important means to analyse the relation between DICOM entities. DICOM abstracts a information model that includes patient, study, series and image level from the relationship between medicine image and patient. These four levels interrelate and interdepend each other. When we deal with medical image which accord with DICOM standard, primary problem is how to get the data of the image file. So it is an important thing to understand data format and file format. DICOM data format is a kind of store means of data, and it is a foundation to understand DICOM file. The general software to process image does not support the medical image file , so it is important for reading and writing and processing DICOM image files to understand correctly DICOM file format.The transmission of the medical image file is an extremely important function of PACS. This paper has studied the transmission of the medical image file on the basis of level model of DICOM network.DICOM is a standard in the field of medical image system, and also is a technological foundation of developing new model medical equipment with inter-connection and sharing information resource . It is the most difficult and important to implement the store and communication of medical image. When developing in the upper level of communication protocol, developers can use communication protocol , and don't involve the concrete hardware. So DICOM keep independent with the development of the network technology. The performance of DICOM can improve immediately with the improvement of the performance of the network. DICOM can transfer medical image efficiently.We design three levels according to DICOM network level to implement communication of medical image. The upper layer includes the DICOM entity, the middle level includes DICOM Message Service Element and Service-Object Pair. Low-layer includes some class which support ommunication.After having designed 3 layers to implement communication of medical image, this paper show how to implement mid-layer with an example of storing service of CT image. Low-layer is based on TCP/IP, and socket is used. At the same time the detail about how to create association, transfer data and close association after transmission. Because the medical image files are usually great, efficiency of communication and storage is relatively low. So compression is needed in order to communicate and store efficiently. This paper introduces some kinds of compression method suitable for DICOM. And this paper expatiates compressing and decompression with an example of run length coding. DICOM picture is the core of DICOM. Except that the storage and communication of image introduced before, the correct display of image is also an important part. Correct display can affect the understanding and diagnosis of doctors. The effective process can stress some valuable part, and amend the display result of image.
