This paper mainly studied the PACS(Picture Archiving and Communication Systems) system to follow the standard of DICOM(Digital Imaging and Communication of Medicine and DICOM) and 3D reconstruction of medical image.This paper can be divided into two parts, the first part introduces the images format and communication protocol of the DICOM image; The second part introduces the implementation of several modules of PACS system in detail, including implementation and class diagrams of PC and Android image browser, back-end services procedures and 3d reconstruction module, and introduces function of each part along with screenshots of the software.The function of PC image browser included adjusting window, measuring, filters, scaling, grid and remote retrieval, in addition to these basic functions, also added on the 3d reconstruction which provides the surface rendering, image division of surface rendering and volume rendering. Android image browser achieved adjusting window, filter, scaling and remote retrieval function, which is the subset of PC image browser. Backend server application implemented four basic communication function of DICOM which are the verification, storage, query and move, and it also has a simple access control function, which can provide the corresponding services for image browser.This paper proposed a method about the adjustment of window width and window center in the interest area of medical image. First, user specify an interest area, then the algorithm can automatically calculate the best window width and window center, which changed the traditional way of manually adjusting window.This system was developed by C#, C++ and Java. C# implemented the interface of PC image browser and server program. C++ implemented the parsing and communication of DICOM files, image processing and 3D reconstruction. Java implemented the interface of Android image browser. This system used Mysql as database, at the same time used open source library like DCMTK and VTK for development.At present, the system has been used in the two-way referral system in hospitals, providing the medical image browse and retrieval function, which greatly improve the efficiency of the doctor processing medical image.
