Clinical Observation Of Huayu Qingre Tongfu Treatment To The Symptom-complex Of Stagnant-heat In The Interior And Gas Of Intestines Emphraxis In The Patients Of AECOPD Combined SIRS

Objective:To observe the improvement of symptoms and signs that Xuebijing Injection and Bianmishu Capsule in patients with AECOPD combined SIRS;To observe the effects on coagulation functions and endotoxin levels in plasma of the two drugs in patients with AECOPD combined SIRS;Evaluation the curative effects of Huayu Qingre Tongfu Treatment to AECOPD combined SIRS.Methods:By the computer random number table,60 patients diagnosed the symptom-complex of stagnant-heat in the interior and gas of intestines emphraxis in the patients of AECOPD combined SIRS in our Pneumology and Critical Care Medicine Division from December 2008 to January 2010 into two groups:The treatment group and control group both 30 cases. The control group treated with conventional Western medicine(see"Chronic obstructive pulmonary disease diagnosis and treatment guidelines" 2007 revised edition), the treatment group on the basis of the control group was added Xuebijing 50ml+saline injection 100ml,iv gtt,2/day for 7 days; Bianmishu Capsule 0.6g, oral,1/night for 7 days. Respectively, to detect the serum endotoxin (ET) level by kinetic nephelometry on the day before treatment and on the 4th,8th day after treatment,and to compare the change between the two groups before and after treatment with symptom score,body temperature(T),pulse(P),respiratory rate(R),WBC,Fibrinogen (Fbg), plasma D-dimer (D-D), the total number of defecation. All the data are used SPSS13.0 processing.Results:1. The symptom score,T,P,R,WBC decreased in both groups after treatment, the difference was statistically significant (P<0.05), and the respite symptom score of treatment group decreased more significantly compared with the control group (P<0.05).2. Both groups Fbg, D-D levels of after treatment were decreased compared with before treatment, the difference was statistically significant (P<0.05); and the treatment group decreased more significantly than the control group (P<0.05).3. After treatment, the ET level of treatment group patients decreased significantly (P <0.01), and the ET level in patients of the control group also decreased, but no statistically significant (P>0.05).4. Compared with the control group, the total number of defecation of the treatment group after treatment increased significantly, and the difference was significant (P<0.01).Conclusion:1. There were some good effect to improve the patient's condition and prevent disease progression by use Xuebijing injection combined with Bianmishu Capsule in the symptom-complex of stagnant-heat in the interior and gas of intestines emphraxis in the patients of AECOPD combined SIRS.2. The mechanism of Xuebijing Injection and Bianmishu Capsule for the symptom-complex of stagnant-heat in the interior and gas of intestines emphraxis in the patients of AECOPD combined SIRS may be related to improvement of blood coagulation and endotoxin removal or inactivation,to reduce the absorb of gut-deriving endotoxin.3. The combination of Xuebijing Injection and Bianmishu Capsules is a reflection of Huayu Qingre Tongfu Treatment, and explain Huayu Qingre Tongfu Treatment does have good curative effect for the symptom-complex of stagnant-heat in the interior and gas of intestines emphraxis in the patients of AECOPD combined SIRS.
