Purpose:Use the technology of metabonomics to clarify the natures of Yin deficiency and Yang hyperactivity syndrome and excessive accumulation of phlegm-dampness syndrome of hypertension. Make a comparison between the two natures to investigate the differences of them and explore the biomarkers which can be used to distinguish the two syndromes. Provide a new basis for syndrome differentiation of hypertension.Methods:According to the guiding principles of Chinese medicine research, choose 20 patients who were diagnosed as hypertension due to Yin deficiency and Yang hyperactivity,12 patients were belong to syndrome of excessive accumulation of phlegm-dampness of hypertension and 22 healthy people who passed the physical examination in the Physical Examination Center of Jiangsu Provincial TCM Hospital. Observe the vital signs and related biochemical indexes and save the plasma, passed high-speed centrifugation, which from the venous blood of the subjects who are on empty stomach for 12 hours in the-80℃refrigerator. As soon as the samples to be collected, conduct the principal component analysis and get the metabolic fingerprinting. After pretreat and integrate the, metabolic fingerprinting, analyze the integral value diversely and statistically. According to the principal component scores of all samples, describe two-dimensional score plot and spatial distribution which are used to determine differences between the two groups. Then filter and determine the biomarkers group which can distinguish Yin deficiency and Yang hyperactivity syndrome and excessive accumulation of phlegm-dampness syndrome of hypertension and the biomarkers group can differentiate the two syndromes from healthy ones.Results:①Compare Yin deficiency and Yang hyperactivity group with excessive accumulation of phlegm-dampness group, there was no significant difference in age, BMI, SBP, DBP, AST, ALT, BUN, Scr,Glu,UA,TC, TG, LDL-c and HDL-c.②There are differences in metabolic products of the principal components between Yin deficiency and Yang hyperactivity group and excessive accumulation of phlegm-dampness group. The different compounds are fructose,citrate, lactate, ethanolamine.③In the group of Yin deficiency and Yang hyperactivity, citrate, inositol, methylglucopyranoside, tryptophan, fructose and octadecatrienoic acid are significantly different from those in normal control group. Citrate is the extreme significant difference between the two groups. But there is no difference of those compounds between excessive accumulation of phlegm-dampness group and normal control group.④Compared with normal control group, there is a significant difference in uric acid in excessive accumulation of phlegm-dampness group, but not in Yin deficiency and Yang hyperactivity group. Conclusion:There are differences in biomarkers group between Yin deficiency and Yang hyperactivity syndrome and excessive accumulation of phlegm-dampness syndrome of hypertension, but they are not sufficient for the identification of the two syndromes. Yin deficiency and Yang hyperactivity syndrome is major and closely related to energy metabolism and lipid metabolism. Uric acid may be one of the potential biomarkers of excessive accumulation of phlegm-dampness group. Metabonomics might provide a clear physical basis for diagnosing and distinguishing the syndromes of TCM of hypertension in the future.
