Analysis On Related Factors Of Depression In Elderly Patients With Chronic Obstructive Pulmonary Disease

Objective:To investigate the incidence rate of depression and related factors in elderly patients with chronic obstructive pulmonary disease (COPD) at stable stage offer provide guidance for their comprehensive clinical treatment.Methods:Three hundred and eleven COPD stable patients cases from Respiratory Department of eight different hospitals in Tianjin were enrolled in this study, the Geriatric Depression Scale(GDS) and the Social Support Rating Scale (SSRS) were investigated to collect the information of patients. Patients surveyed were all in compliance with GOLD'S standard and the diagnosis and treatment of row norms for patients with chronic obstructive pulmoary disease of the the Chinese Medical Association (All subjects who accepted the survey were on a voluntary basis) voluntary survey.Patients with serious intellectual and cognitive impairment, confusion, mental retardation, dementia, intubation, drug and alcohol dependence, deaf, and speech communication were excluded from the study.All patients are were elder than sixty years. The SPSS 11.5 statistical software was used for data analysis.Results:(1)The incidence rate of depression in the patients of COPD at stable stage was 51.2%, patients with mild and moderate to severe depression accounted for 38.3% and 12.9% respectively. (2) The percentage of female patients suffered from depression was more than the percentage of male patients, the proportion of female to male was 3:2, which reached the statistically significance level. (3) Analysis showed that the depression groups are significantly associated with the COPD scale,rank and social support.When grouped(or stratified) by smoking status,respiratory faliure history and medical coverage sources, statistically significant differences were observed for the COPD patients with depression (p<0.05). (4) After analysis, the GDS scores are related to gender, smoking status, the number of cigarettes smoked, years of smoking, cough, sputum, wheezing age, the number of complications,COPD classification,whether there had respiratory failure, medical expenses, sources of social support scores, FVC measured value, FEV1/FVC ratio and other statistically significant. (5) Factors included in the multiple regression equation were as follows:gender (β=0.167, P<0.05), the number of cigarettes smoked (β=0.142, P<0.05), years of sputum (β=0.172, P <0.05),source of medical coverage(β=0.123, P<0.05), total score of social support (β=-0.162, P<0.05) and support the utilization scores (β=-0.291, P<0.05).The multiple regression equation for the depression was:Y(depression)= 1.776-0.069×support the utilization of social support scores-0.014×total of social support scores +0.009×age+0.171×cost sources of sputum+0.169×Sex+0.006×number of cigarettes smoked.Conclusions:Elderly patients with stable COPD have had a higher prevalence of depression.The percentage of elderly female patients suffered from depression was more larger than the percentage of elderly male patients.And the incidence of depression was in positive correlation with the patients' gender, smoking status,sputum status between age,total degree of social support and support utilization was negatively related to scores.Our study suggests that attentions must be paid to the existence of psychological problems in COPD patients when their physical illness are being treated at the same time,further studies must be done to investigate the intrinsic relationship between psychological and physical problems of the COPD patients. And the treatment of physical diseases of the COPD patients should be combined with psychological counselling and intervention.For patients who also suffered from moderate to severe depression,anti-depressant therapy is adivised when it is necessary.
