Purpose:Makeing use of needling of opposite side and routine acupuncture to treat acute sprain of ankle, according to the improve situation of clinical symptoms and signs before and after treatment,as well as the disappearance of pain in terms of time to compare the effect of treatment between needling of opposite side and routine acupuncture for treating acute sprain of ankle, evaluate the effect of needling of opposite side treating acute sprain of ankle, to facilitate a better clinical methods for treating the disease.Method:Choose the patients from outpatient of acupuncture office of Hubei TCM Hospital. Let the 72 cases of acute sprain of ankle who meet the diagnostic standard be divided into two groups randomly, the treatment group and comparative group both are 36 cases.The treatment group using needling of opposite side, determine the pain points of sprained joints first, and then looking for a clear pain point in the opposite wrist tendermess,after found the tender point,press the point for two minutes first, at the same time, ask patient to move the suffering joint, and stay in the most painful position as far as possible. After the press, put the needle into the tender point at the same time let patient have a cough, to move about the needles every 15 minutes, at this time ask patient to move the suffering joint, and remain for 30 minutes. Treat once a day, ending after 7 times. Comparative group using routine acupuncture,mainly choose jiexi,kunlun,shenmai,zhaohai,qiuxu,ashixue and so on, all the acupoints from the sprain side. Also remain for 30 minutes. Treat once a day, ending after 7 times.Score the indicators of patient's symptoms, signs and the time of pain disappearance, respectively, before treatment, after treatment for 3 times, and after the end of treatment, count the diversification of the integral, and to comparise the clinical effect.Evalution method:By using the scores can range of clinical.symptoms and signs classification, to mark the score respectively before treatment,after three time's treatment and the end of treatment, then calculated the efficacy index of patients, and defined the results according to the standard of efficacy: efficacy index≥95%, recovered; efficacy index≥70%, effectual; efficacy index>30%, improvement; efficacy index<30%, no effect.Using fuzzy visual analogue scale (VAS), to enable patients mark score themselves according to their degree of pain, then to compare the difference of effect in the two treatment method according to the changes of points before and after treatment.Results:1.It's more effective that the method of needling of opposite side compared with the comparative group in eliminating pain,improve the symptoms and reduce the degree of swelling after the treatment of first three times(p<0.05),but the difference of effect was not statistics significant after a full course of treatment (p>0.05).2. The difference exist in cure rate,the total efficiency and the overall efficacy both after the treatment of the first three times and after a full course of treatment, but the difference was not statistics significant after the statistical test.3. The needling of opposite side in the treatment of acute sprain of ankle compared with the routine acupuncture, there is only a little difference,but the onset of needling of opposite side faster significantly. Conclus ion:The results show that making use of needling of opposite side treat the acute sprain of ankle,with the principle of selecting points according to "thorn venae if the disease in network", this reflects the needling of opposite side's characteristics of rapid onset of pain relief, it's also a very good effect for the other clinical symptoms, signs and other improvements,and selecting less points, it's accepted easier by patient. As the number of sample patients is too little,and the observation is limited to the acute phase, so lack the statistics of the recovery time complete in patients, the following we must to do is designing more regorous program, expand the number of sample patients, to carry out clinical trials of massive and long-term observation,for the traditional acupuncture can treat disease based on medical theory, also explore the mechanism of action of needling of opposite side is used in treating acute sprain of ankle further.
