| Objective:To investigate the the factors that expectant mother select cesarean delivery (CD) as the primiparity choice by retrospectively analyzing clinical data of puerperant and newborn during three years in our hospital and to explore the possibility to assess the quality of obstetrics by the risk-adjusted primary CD rates.M ethods:All the birth records were analysed retrospectively between January 1,2006 to January 1,2008 in our maternity hospital and the correlation factor of selecting the CD as the primiparity choice which in-clude parity,birth age, ethnicity, occupation, mother's educational level,whether for checkups during pregnancy, birth gestational age, multiple pregnancy, pregnancy complica-tions,and intrapartum complications by using a logisic regrcssion mode. Results:1.The risk factors of CD include:multiple pregnancies, intrapartum complications, complications during pregnancy, childbirth, age, mother's level of education and the protective factors of CD include:ethnic minority people, premature birth in our maternity hospital.2.The logisic regrcssion mode of risk-adjusted primary CD in our hospital is Y=2.146+1.368 multiple pregnancy+3.122 breech or transverse+0.797 moderate+1.125 severe complications in pregnancy complications in pregnancy+0.979 mothers older than 35 years of age+0.134 mother's level of education-0.197 ethnic minority people-1.111 preterm birth by maternal-0.302multipara.3.The risk-adjusted CD rates is approaching the actual CD rates, and the trend was constantly close during the three years.Conclusion:1.Of multiple pregnancies, intrapartum complications, complications during pregnancy,childbirth, age,mother's educational level will lead to increasing rates that expectant mother select CD as the primiparity choice.2.The logisic regression mode of risk-adjusted primary CD can effectively assess quality of obstetrics and provide scientific basis for controling the right CD rates.3.The differences between the actual CD rates and the risk-adjusted CD rates gradually decreased,reflecting improved quality of obstetrics at our hospital. |
