Gamma Knife Treatment For Endocrine Pituitary Adenoma

Objective To investigate the effect of gamma knife radiosurgery for endocrine pituitary adenoma and to analyze the best treatment project. Methods Retrospective analysis of 92 cases of Gamma Knife treatment information in patients with endocrine pituitary adenoma complete clinical data, male 34 cases, female 58 cases,56 cases of prolactinoma,20 cases of growth hormone adenoma, ACTH adenoma in 6 cases, Mixed hormone adenoma 8 cases.23 patients were treated by Bromocriptine before Gamma Knife treatment, tumor was classify according to tumor size, type, invasive, and use the OUR-XGD Gamma Knife treatment.50%isodose. curve enveloping the target, given marginal dose 14-30Gy, for a huge adenoma using fractionated treatment, the efficacy was evaluate by the postoperative hormone levels,imaging study and clinical symptoms. Results 92 cases were completely followed up 6～36 months, an average of 18.5 months, the restoration of normal hormone levels in 46 cases,compared with the previous decline＞50%but still higher than normal 30 cases, but lower than the previous decline in 50%about 5 cases, no change in those 11 cases, the total effective rate 88.4%; 38 cases of the disappearance of tumor,45 cases of narrow, no change in 6 cases,3 cases of increasing, there is efficiency of 90.2%; clinical symptoms completely disappeared in 30 patients,42 cases of remission, no change in 18 cases, 2 cases of increase, there is efficiency of 78.3%.5 cases recurrence in 1～2 years after treated by gamma knife, there are Low pituitary function in 4 cases, the efficacy of gamma knife treatment for endocrine pituitary adenoma is related with tumor size, invasive, and there is statistical significance in the history of surgery, and the preoperative hormone. Conclusion Gamma knife radiosurgery for endocrine pituitary adenomas are safe and effective method,, its efficacy is relevant with tumor size, medication history, and preoperative hormone levels. It's long term curative effect and complications still need further observation.
