The Effect And The Related Factors Analysis Of Gamma Knife Radiosurgery For Pituitary Adenoma

Objective To investigate the effect of gamma knife radiosurgery for pituitary adenomas and to analyze the related factors that affect the efficacy.Methods Retrospective analysis of 238 cases of Gamma Knife treatment information in patients with pituitary adenoma complete clinical data,male 90 cases,female 148 cases,98 cases of prolactinoma,60 cases of growth hormone adenoma,ACTH adenoma in 18 cases,Mixed hormone adenoma 16 cases,46 cases of nonfunctioning adenomas. 12 patients were treated by surgery before Gamma Knife treatment,tumor was classify according to tumor size,type,invasive,and use the OUR-XGD Gamma Knife treatment.50%isodose curve enveloping the target,given marginal dose 14-30Gy,for a huge adenoma using fractionated treatment,the efficacy was evaluate by the postoperative hormone levels,imaging study and clinical symptoms.Results 238 cases were completely followed up 6-36 months,an average of 18.5 months,the restoration of normal hormone levels in 98 cases,compared with the previous decline> 50%but still higher than normal 58 cases,but lower than the previous decline in 50%about 12 cases,no change in those 24 cases,the total effective rate 87.5%;98 cases of the disappearance of tumor,120 cases of narrow,no change in 16 cases,four cases of increasing,there is efficiency of 91.6%;clinical symptoms completely disappeared in 78 patients,112 cases of remission,no change in 42 cases, six cases of increase,there is efficiency of 79.8%.16 cases recurrence in 1-2 years after treated by gamma knife,there are Low pituitary function in 14 cases,the efficacy of gamma knife treatment for pituitary adenoma is related with tumor size,invasive,and there is statistical significance in the history of surgery,and for functional pituitary adenomas also with the preoperative hormoneConclusion Gamma knife radiosurgery for pituitary adenomas are safe and effective method,its efficacy is relevant with tumor size,invasive and surgical history,and for functional pituitary adenoma with preoperative hormone levels.
