Cost Analysis Of In Treatment Of Lumbar Disc Herniation

Objective:To analyze the costs of decompression with and without fusion in the treatment of Lumbar disc herniation. Methods:The selected patients of lumbar disc herniation were operated in a hospital from Jan.2005 to Oct.2007.851 received decompression and 242 received decompression with fusion. The length of optration, intraoperative blood loss,postoperative stay and off-work day, hospital charge, and total costs were analyzed and compared. Results:The average duration of operation in decompression group(min)was significative less than that in decompression with fusion group(min)(P<0.05). The former is 71.55±33.19min, The latter is 113.14±73.95min;The average intraoperative blood loss in decompression group(ml)was significative less than that in decompression with fusion group(ml)(P<0.05). The former is 218.45±89.91ml, The latter is 517.43±334.30ml;The average duration of postoperative antibiotic in decompression group(d)was no significative compared with fusion group(d)(P>0.05). The former is 7.76±2.69d, The latter is 7.86±3.19d; The average of postoperative stay and off-work day in decompression group(d)was significative less than that in decompression with fusion group(d)(P<0.05). The former is 14.12±6.16d, The latter is 16.04±6.20d; The average costs of operation in decompression group(yuan)was significative less than that in decompression with fusion group(yuan)(P<0.05). The former is 986.56±3244.15yuan, The latter is 1940.63±509.60yuan; The average material costs of operation in decompression group(yuan)was significative less than that in decompression with fusion group(yuan)(P<0.05). The former is 37.28±51.03yuan, The latter is 15670.92±13833.64yuan; The average medical costs in decompression group(yuan)was significative less than that in decompression with fusion group(yuan)(P<0.05). The former is139.32±478.27yuan, The latter is 2365.12±1835.64yuan; The average costs on beds in decompression group(yuan)was significative less than that in decompression with fusion group(yuan)(P<0.05). The former is 44.27±12.76yuan, The latter is 666.89±446.21yuan; The average costs on hospitalization in decompression group(yuan) was significative less than that in decompression with fusion group(yuan)(P<0.05). The former is 1452.42±1245.92yuan, The latter is 23479.94±15240.3yuan.Conclusion:The main influence factors of hospitalize costs are material costs, medical costs, operative costs and beds costs. Under the appropriate effectiveness, decompression then fusion are certain of increase the duration of operation, intraoperative blood loss, postoperative stay and off-work days and the total costs.
