Objective:To compare the clinical effects of percutaneous endoscopic lumbar discectomy(PTED)and transforaminal lumbar interbody fusion(TLIF)for the treatment of single segment lumbar disc herniation.Methods:A retrospective study was made of 64 patients with LDH in October-2016 March 2015.The medical data of all the patients were complete.According to the surgical procedures included in the patients,32 cases were divided into group PELD(18male /14 femal)and 32 cases in group TLIF(16 male/16 female).General information: in group PELD,the average course of disease was 13.8 months,3 cases of lesion segment L3-4,20 cases of L4-5 and 9 cases of L5-S1.The average course of disease in TLIF group was 14.2 months,the lesion segment L3-4 5 cases,L4-5 18 cases,L5-S1 9 cases.The clinical efficacy was evaluated by visual analogue scale(VAS),Oswestry disability index(ODI)and Mac Nab curative effect evaluation standard,and the operative complications of the two groups were recorded.The collected data were statistically processed with SPSS 12 software,and the measurement data were expressed with((?)ąs).The two groups were evaluated by chi square test for the evaluation of the effect of Macnab.The VAS score and the ODI score were tested by independent sample t test.The difference between P < 0.05 was statistically significant.Results:All the patients in this study were operated smoothly,and the follow-up data were complete before and after the operation.There was no significant difference in preoperative VAS score between the two groups(P > 0.05).VAS score in PELD group and TLIF group was significantly lower than that before operation(P<0.05).In the two groups,the VAS score of the same follow-up time was compared,and the score of group PELD was significantly lower than that of group TLIF(P<0.05).There was no significant difference in ODI score between group PELD and group TLIF before operation(P > 0.05).After the operation,the ODI scores of all the two groups were less than those before the operation,but there was no significant difference between the groups(P > 0.05).The good rate of evaluation of Macnab in group PELD and group TLIF was 96.8% and 93.8% respectively,but there was no statistical difference between the groups(P<0.05).In group PELD,the length of operation,the amount of bleeding,the flow rate after operation,and the time of hospitalization were significantly smaller than that of the group TLIF(P<0.05).Conclusion:percutaneous endoscopic lumbar discectomy surgery(PELD)and transforaminal lumbar interbody fusion(TLIF)can achieve good clinical efficacy in the treatment of LDH patients.But percutaneous transforaminal surgery has advantages in reducing postoperative lumbago,and has advantages in operation time,bleeding volume,drainage volume,hospitalization time and operative complications.
