Objective: Adiponectin gene (Apm-1) polymorphism has been one of genetic susceptibility to polycystic ovary syndrome (PCOS) and non-alcoholic fatty liver disease (NAFLD) in recent years. To access the correlation between single nucleotide polymorphisms (SNP45 and SNP276) of adiponectin gene and nonalcoholic fatty liver disease in polycystic ovary syndrome in Han Chinese people by detecting the second exon 45 and the second intron 276 polymorphism of adiponectin gene, the following experiments were performed: To explore the association of Apm-1 polymorphism and clinical factors with nonalcoholic fatty liver disease in polycystic ovary syndrome , To investigate the relationship between the Apm-1 276G>T,Apm-1 45T>G polymorphism frequency and serum Adiponectin levels. Thereby, it may provide a theoretical basis for the monitoring of susceptible crowd and early prevention of NAFLD in PCOS .Methods: A case-control study was carried out at Tang Shan Healthcare Hospital for Women & Children from March 2008 to September 2009. Totally 100 subjects of NAFLD in PCOS were selected as Case group , 100 subjects of PCOS were selected as controls in Han Chinese people. Acquired information including age, waist circumference, hip circumference, height, weight, smoking etc; The biochemical parameters were measured by automatic biochemical analyzer; The serum insulin levels were determined by Radioimmunoassay; Reproductive hormones were measured by chemiluminescence immunoassay ; Serum adiponectin levels were determined by ELISA; The Apm-1 45T>G, 276G>T polymorphisms were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). All statistics were performed using Statistical Analysis System software SPSS16.0.The measurement data was analyzed by x2-squared test, t-test ,one-way ANOVA and multivariate unconditional logistic regression analysis according to data type.Results: The frequency of Apm-1 45T>G polymorphism and that of Apm-1 276G>T polymorphism were concordant with the Hardy-weinberg balance fixed rule.There was no significant difference for subjects ranged in age and BMIWaist circumference, waist to hip ratio, CHOL, TG, LDL and IR index in the case group was significantly higher than the controls and was risk factors for NAFLD in PCOS , whereas the serum adiponectin and HDL levels in the case group were significantly lower than the controls and were protective factors for NAFLD in PCOS .The distributions of genotypes and alleles of Apm-1 SNP45 in the case group / the controls were as follows: TT 71%/72%, GT 25%/20%,GG 4%/8%, T 84%/82%,G 16%/18%. No significant difference in genotypes and alleles (P>0.05)of Apm-1 SNP45 between two groups was found. The distributions of genotypes and alleles of Apm-1 SNP276 in the case group / the controls were as follows: TT 8%/16% ,GT 36%/51%, GG 56%/33% ,T 26%/42%,G 74%/58%. Significant difference in genotypes and alleles (P<0.05= of Apm-1 SNP276 between two groups was found. The presence of SNP276TT+GT carried a 0.387-fold increased risk(95%CI: 0.218-0.687, P<0.05)of NAFLD in PCOS.No difference in serum adiponectin levels was observed among the three genotypes of SNP45 between two groups(P>0.05). Subjects with NAFLD in PCOS carrying the SNP276TT genotype had higher serum adiponectin levels. Logistic regression showed that serum adiponectin levels, HDL and Apm-1 276T allele were all independent protective factors for NAFLD in PCOS, while IR index,TG, LDL, Waist-Hip ratio and Waist was all independent risk factor for NAFLD in PCOS. However, no association of the Apm-1 45T>G polymorphism with NAFLD in PCOS.Conclusions: The research indicates that (1) The adiponectin gene 276G>T polymorphism is significantly associated with the presence of NAFLD in PCOS and The allele of adiponectin gene 276T is independent protective factor for the incidence of NAFLD in PCOS. The relationship between the adiponectin gene 45T>G polymorphism and the incidence of NAFLD in PCOS can not be observed. (2) The adiponectin gene 276G>T polymorphism has an effect on expression of serum adiponectin levels and subjects carrying the 276T allele have higher serum adiponectin levels. Adiponectin gene 45T>G and serum adiponectin levels are not related.
