Objective: To detect the changes in the level of H-FABP and to explore the possible clinical value of serum H-FABP in patients with chronic heart failure.Methods: 63 patients with chronic heart failure in different Heart function (CHF group) and 20 healthy people in physical examination over the same period (control group) were collected, CHF group was divided into 3 subgroups(gradeⅡ,ⅢandⅣ)according to the NYHA functional class standard, serum H-FABP,cTn T,Mb,and NT-proBNP were detected in all selected candidates,to do analysis of the positive rate and compare the concentration difference of each biomarker between the CHF group and control group and between the subgroups , make correlation analysis between H-FABP and NT-proBNP finally. All statistical analysis completed in SPSS 17.0.Results: Compared with the control group, H-FABP and cTn T level in CHF group were higher [(5.18±1.58 vs 2.61±0.82) ng / ml, (0.56±0.05 vs 0.02±0.01) ng / ml, P<0.05]; Mb and NT-proBNP levels were compared with the control group significantly different [(156.0±231.0 vs 50.7±23.8) ng / ml, (1145±1748 vs 139±163)pg/ml, P <0.05]. The overall positive rate of H-FABP in CHF group (58.7%) is higher than cTn T (44.4%) (P <0.05), while lower to Mb (68.3%) (P <0.05); in severe cases of heart failure (III + IV stage Level group), H-FABP and Mb are similar in positive rate, significantly higher than that of cTn T (P <0.05) [respectively 76.2%, 79.1%, 58.1%]; joint analysis of the three indicators of the positive rate was 92.1%. From the grade II to IV,H-FABP and NT-proBNP level was gradually increased, there was significant difference between subgroups (P <0.05), but cTn T and Mb have no regular trend. Linear correlation analysis showed: H-FABP and NT-proBNP was positively correlated (r = 0.874, P <0.01).Conclusion: CHF patients may have ongoing myocardial damage (OMD); H-FABP can be used for the detection of CHF patients with OMD and its detection sensitivity is higher than cTn T but similar to Mb; the discovery of OMD is more sensitive in combined detection of H-FABP, cTn T and Mb. H-FABP has important clinical value in judging the severity and guiding the NYHA classification of CHF patients.
