| Objective : To study the serum level of heart-type fatty acid- binding protein (H-FABP) in children with Congestive heart failure (CHF) and assess the value of H-FABP.Methods: The serum levels of H-FABP ,Cardiac troponinâ… (cTnI) and Creatine kinase -MB(CK-MB) were determined in the children with CHF(n =56) at the heart failure and recovery stage and in the control group (n=40) respectively by enzyme-linked immnoabsorbent assay. The ejection fraction (LVEF) of CHF group were measured by two-dimensional echocardiography at the heart failure and recovery stage.Results: (1) The serum level of H-FABP and cTnI in CHF group was significantly higher than that in the control group (p< 0.01 and 0.05 respectively), there was no significant difference in serum CK-MB concentrations between the two groups (p>0.05). (2) The H-FABP concentration in CHF group at the recovery was markedly higher than that in control group (p<0.01). (3) The H-FABP concentration of heart failure group from different disease were all higher than that in the control group, and that of different disease were no significant difference (p>0.05 all). (4) The H-FABP concentration in CHF group at the heart failure stage was markedly higher than that at the recovery stage (p<0.01), the cTnI concentration was higher than that (p<0.05), while there were no significant change of CK-MB concentration between the two stages (p>0.05). (5) The difference of the H-FABP concentration with heart functionâ… ,â…¡,â…¢were all markedly (p<0.01 all). (6) The H-FABP concentration in CHF group at the heart failure had negative correlation to LVEF (r=-0.83,p<0.01). (7) The abnormal rate of H-FABP and cTnI in CHF group were markedly higher than that in the control group (p<0.01 both), the level of CK-MB concentration had no significant difference between the two groups (p>0.05), and the difference of the abnormal rate between H-FABP and cTnI was markedly (p<0.01).Conclusion: The serum concentration of H-FABP and cTnI can be regarded as the biomarker of myocardial damage in the children with CHF, and the H-FABP has more sensitivity and specificity; the serum level of H-FABP is closely related to the severity of myocardial damage in the children with CHF. The serum H-FABP concentration is a guideline of early diagnosis, clinical treatment and prognosis in the children with CHF. |
