To Observe The Clinical Effect On Functional Dyspepsia By Catgut Implantation On Acupoint

BackgroundFunctional dyspepsia (FD) is a kind of disease which is common and frequently-occurring; it refers to a group of clinical syndromes which includes sustained or repeated abdominal distention, pain and early abdominal fullness excepting physical diseases. Epidemiological investigations revealed that functional dyspepsia is widespread. Almost everyone has had symptoms of dyspepsia in their life, the only difference between people being duration and the influence on their lives. About 20-40% of the population has had the symptoms of dyspepsia. Therefore, diagnosing and treating FD accurately is very important for people's health, and researching effective ways to treat FD has significant and practical economic and social meaning.There are many kinds of methods to treat FD which includes drugs, Chinese herbal medicine, acupuncture, psychology and so on. Each method has its drawbacks:pharmacological treatment has many side effects in later periods, mental healing is too expensive to be popular, and there is no criterion for diagnosis, treatment and therapeutic evaluation in Chinese medicine. Acupuncture therapy, on the other hand, has many advantages:no side effects, lower charges, and so on. But the effective time period of acupuncture is short, and it is not convenient to make patients receive acupuncture treatment every day or every other day. Catgut implantation at acupoint is a treatment that implants the catgut into specific acupuncture point, and it can keep a long-term stimulation for acupuncture point, extend the effective period of acupuncture, reduce the frequency of doctor visits.ObjectiveThe study was to observe the effect of catgut implantation at acu-point on the treatment of FD and to discuss the therapeutic mechanism, and to appraise its therapeutic evaluation on the treatment of FD. It was also expected to obtain the basis of evidence based medicine for catgut implantation at acu-point on treating of FD, and then to improve the clinical effect of TCM on treating FD. This method can then improve the quality of patients'lives and achieve economic and social efficiency.MethodsIn this study, we chose and treated 60 patients with FD from the Digestive System Dept. and Acupuncture Dept. of First affiliated hospital of Guangzhou University of TCM. Those who have met the criteria were randomly divided into two groups:the treatment group and the control group.Each group had 30 patients. The treatment group was treated by catgut implantation at acu-point with Chinese herbs, while the control group was treated by Chinese herbs only. and both groups were treated for 3 periods. The acupoints of treatment group were Zhongwan, Tianshu, Guanyuan and Zusanli.We observed two groups' clinical effect with Symptom rating scale of FD and SF-36. We assessed the difference of clinical effect between the two groups by SPSS software statistics.ResultsAfter the treatment, the effective rate of the treatment group and control group were 90.0% and 76.7% respectively. The difference of clinical effect between two groups had no remarkable significance (P>0.05) after statistical analysis. Two groups'score difference of Symptom rating scale of FD between before and after treatment have remarkable significance (P<0.05). But through comparison with two groups'score of after treatment, we find that the symptoms'change of gasteralgia (P=0.032<0.05), abdominal distention after food (P=0.002<0.05), early abdominal fullness (P=0.034<0.05) and irritable (P=0.030<0.05) have remarkable significance.About two groups'quality of patients'life, we observed the two groups' change between before treatment and after treatment from eight parts:Physical Function, Role Physical, Bodily Pain, General Health, Vitality, Social Function, Role Emotion and Mental Health. Through comparison of the two groups'scores of "after treatment" with "before treatment", we find every part's P less than 0.05, except control group's Vitality (P=0.057>0.05). After comparison of the two groups'quality of patients'life after treatment, the results show us that the two groups'changes have remarkable significance in Physical Function, General Health, Vitality, Social Function and Mental Health (P<0.05). Therefore, the treatment group is more effective than control group.ConclusionClinical research showed us that catgut implantation at acu-point with Chinese herbs has better effect on the improvement of symptom and quality of life than Chinese herbs only.
