Objective:To explore the effect of a community-based pulmonary rehabilitation program to the life quality and pulmonary function of the patients with stable chronic obstructive pulmonary disease(COPD).Method:The participants were selected from the patients with stable COPD above stage II residing in Changhua Street Community, Liwan District, Guangzhou City. All of the 216 participants were given the routine exams including items such as the spirometry, a 6-minute walk, Borg index and St. George's Respiratory Questionnaire (SGRQ), and they all conformed to the clinical diagnosis criteria established by Chinese Medical Association(CMA) in 2007. The participants were divided into 3 groups according to the data of FEV1/FEV1%:Group A (COPD stageⅡ) was 76 cases; Group B (COPD stageⅢ) was 73 cases; Group C (COPD stageⅣ) was 67 cases. The pulmonary rehabilitation program was carried out within 12 months, consisting of two parts, health education and physical exercise. The health education was conducted by a respiratory physician once every two weeks, which included instructions on the self-supervision, everyday nutrition, psychological adjustments and respiratory exercise, while the physical exercise was carried through twice to three times a week, which included lower limbs exercise and upper limbs exercise under the supervision of the medical staff. The records of the rehabilitation exercises were kept for the physician to examine once a week. Results:1)pulmonary function.After a twelve-month pulmonary rehabilitation training, Group A and B showed indices of pulmonary function were improved,while Group C's were degraded.But the difference was not significant.2)physical mobility.Six-minute walk experiment results: Group A:(401.29±37.34m vs 344.88±36.16m,P<0.001) Group B:(321.32±43.65m vs 259.19±31.25m, P<0.001) Group C:(271.23±23.48m vs 215.47±15.24m, P<0.001) The average result:(333.92±64.63m vs 275.78±61.40m,P<0.001)3)The life quality of the patients with COPD: Group A:(35.98±4.29 39.18±4.48 33.76±6.09 35.76±3.93 vs 51.50±4.44 48.26±5.28 51.05±4.26 50.28±2.86, P<0.001) Group B:(60.19±5.17 59.12±4.98 56.98±4.05 58.28±3.24 vs 66.73±4.83 65.32±5.36 65.29±4.10 65.54±3.10, P<0.01) Group C:(73.42±4.08 69.41±5.41 64.89±3.76 67.70±3.22 vs 77.62±3.96 75.83±6.28 71.53±4.41 73.86±3.41, P<0.01) The average result:(55.77±16.20 55.30±13.53 51.26±14.15 53.28±13.92 vs 65.28±11.60 62.57±12.69 62.22±9.60 62.75±10.28, P<0.05.Conclusions:Positive results were obtained in terms of the physical mobility and life quality of the patients with stable COPD through the rehabilitating experiment.It suggests that the community-based pulmonary rehabilitation program is practically effective.
