BackgroundDepression after stroke as early as 1940s foreign scholars have reported it,but at that time only put the blame on the psychological reaction body dysfunction. Nearly several year of research proof:depression and stroke after stroke has a direct relationship,cerebrovascular disease caused not only the body defect,and with psychiatric disorders,including the most common in depression.On depression after cerebral apoplexy depression,often present as interest reduce, sports is slow,cognitive impairment,lack of self-knowledge force and daily living skills drops.If the symptoms persist more than 2 weeks,named Post-depression PSD.Objectives1. The first discussed in patients with cerebral infarction occurred the PSD time regulation, disease characteristics,related risk factors;2.To discussion accompanies PSD the first time cerebral infarction,with the non-PSD patient compared to the ability of living,the nerve function,again cardiocerebrovascular events and so on.Materials and MethodOf February 2010 to September 2010,178 patients hospitalized patients with first cerebral infarction,use Mini-mental state examination (MMSE) were rating of cognitive function,after the antiforeign cognition function barrier,has 135 examples.For this 135 patients with Hamilton depression scale(HAMD) carries on despondent grading,with the NIH stroke scale (NIHSS) carries on nerve function,Barthel Index capacity Index (BI) scale,carries on daily life quality grading.Then,in 1 months,3 months,6 months after repeated HAMD, respectively,and ask whether again cardiocerebrovascular events,and the incidence of calculating PSD.Results135 cases of patients with cerebral infarction,lost to follow-up 9 cases,the remaining 126 cases,including 64 cases, with an incidence of 50.8% merger depression.Among them,cerebral infarction occurred within 1 month,with an incidence of 44 cases of depression 34.9%,cerebral infarction occurred within three months,with an incidence of 18 cases of depression,cerebral infarction 14.3 percent after six months occurrence depression in 2 cases,with an incidence of 1.6%. The incidence of PSD have significant differences in gender (x2= 349,p= 0.04), women who PSD ratio is higher than male.The fixed by education of PSD compared with the PSD leader,both statistically significant (t=2.81, p=0.046). Patients with cerebral infarction of smoking than non-smokers suffer more from PSD (x2=3.83,p=0.04).The PSD group and the non-PSD group of history of hypertension (x2=3.81,p=0.04) and high blood glucose history (x2=8.97, p< 0.01) statistically significant, a history of hypertension and history of hyperglycemia in patients with cerebral infarction easier comorbid depression. Two groups of patients at different observation of BI statistically significant (F= 2.45 respectively, p=0.048), NIHSS also statistically significant (F=4.16, p= 0.044), the PSD group better than PSD group. The PSD group has 10 people again cardiocerebrovascular events,death 2 people,the non-PSD group have three people again cardiocerebrovascular events,death,l.Comparing the two groups,statistically significant (χ2=0.18,p= 0.047).Conclusion1. The PSD occurrence of time rule:the PSD occurred in the highest after cerebral infarction 1-3 months.2. Related risk factors:women, smokers, who have hyperglycemia history is more likely to occur PSD.3. Treatment outcome:not with depression in patients with cerebral infarction of the ability of living recovery,and neurological recovery are superior in patients with depression,have depression in patients with cerebral infarction of more easily again cardiocerebrovascular events.
