Objective:A comparative study of the treatment effects, that the complex talar neck fractures treat by the fibular osteotomy or non-osteotomy approach.Methods:22 patients were suffered by complex talar neck fracture, who were treated in the Second Xianya Hospital from May 2005 to April 2010. They were divided into two groups randomly:One group of 12 patients received treatment by fibular osteotomy approach; another group of 10 patients got the treatment by non-osteotomy approachl. Follow-up for one year, we evaluated the treatment effect by the VAS pain scores, AOFAS (American Foot and Ankle Society) ankle and hindfoot function score and the complication rate. We analysed the data by application of statistical analysis at last.Results:22 patients were followed for an average of 1.7 years (13 months to 24 months). Osteotomy group VAS pain score (4.03±0.59), non-osteotomy group VAS pain score (5.98±0.77), the two groups were significantly different (p<0.05). AOFAS score osteotomy group (80.56±7.89), non-osteotomy group AOFAS score (69.33±8.24), the two groups were significantly different (p<0.05). Osteotomy group complication rate (41.67%), non-osteotomy group complication rate (60.00%), two groups were significantly different (p<0.05). Conclusion:Complex neck fractures of the talus was treated with fibular osteotomy approach was better than treated with non-osteotomy approach. The former on can reduce the pain feeling and complication rate, it also improve the function of the ill limb.
