The Clinical Research Of Arthroscopic Debridement With Fibular Proximal Osteotomy In The Treatment Of Knee Osteoarthritis

BackgroundKnee osteoarthritis is a common disease of middle-aged and elderly people.The main symptoms of the disease are pain and restricted joint activities.The disease bothers the patient for a long time and the quality of life becomes low seriously.The etiology and pathogenesis of knee osteoarthritis is still not very clear,generally it is believed that damaging the integrity of the cartilage may be the reason of this disease.Its pathological feature is progressive degradation of articular cartilage,until weight-bearing area of full-thickness cartilage disappears.The purpose of the treatment of knee osteoarthritis is relieving the pain,preventing and delaying the progress of the disease,protecting joint function and improving the quality of life.At present,the surgical methods of knee osteoarthritis mainly include arthroscopic debridement,total knee replacement,high tibial osteotomy and fibular proximal osteotomy.ObjectiveTo discuss the curative effect of arthroscopic debridement with fibular proximal osteotomy in the treatment of knee osteoarthritis(OA).MethodsWe retrospectively analyzed 44 patients(44 knees)from August 2015 to October 2016 in our hospital with clinical data.26 patients underwent the arthroscopic debridement therapy(observation group)and 18 patients underwent the arthroscopic debridement with fibular proximal osteotomy(control group).Every patient was graded according to the cartilage lesion under the arthropic visual observation.In observation group,there are 9 cases belonging to grade ? and 9 cases belong to grade ?.Respectively,under the same conditions,there are 12 cases belonging to grade ? and 14 cases belonging to grade ? in control group.VAS score,Lysholm score and tibiofemoral joint space angle were measured between the two groups.ResultsComparing with preoperation,Lysholm score increased,but VAS score and tibiofemoral joint space angle decreased postoperatively.The differences of the two groups between preoperation and postoperation were statistically significant(P < 0.05).There was no significant difference of tibiofemoral joint space angle of the control group between preoperation and postoperation(P > 0.05).But in observation group there were significant differences between preoperation and postoperation(P< 0.05).ConclusionComparing with arthroscopic debridement alone,arthroscopic debridement with fibular proximal osteotomy together can significantly improve function,reduce symptoms and achieve a better curative effect.
