ObjectiveThis randomized controlled double-blind clinical trial intends to observe the effect of acupuncture on Foot Taiyang and Shaoyang Meridians together with Angelicae Pubescentis and Loranthi Decoction to treat ischiodynia and explore its theoretical basis so as to further promote its clinical application.MethodsResearch subjects are 90 patients treated in Zheng Yongfeng's TCM Clinic in Taiwan. These ischiodynia patients who met the inclusion criteria are randomly classified into the Acupuncture and Chinese Herbal Medicine Group, Simple Chinese Herbal Medicine Group and Simple Acupuncture Group with 30 patients in each group by the random classification software according to patients'serial number. The Acupuncture and Chinese Herbal Medicine Group's cardinal prescription is the modified Angelicae Pubescentis and Loranthi Decoction combined with acupuncture on Foot Taiyang and Shaoyang Meridians.Foot Taiyang Prescription:Shenshu,Large Intestine Shu,Huantiao, Zhibian,Weizhong, Yanglingquan,Chengshan and Kunlun.Foot Shaoyang Prescription:Kidney-shu, Large Intestine-shu, Huantiao, Yanglingquan, Xuanzhong and Qiuxu.Acupuncture on Huantiao and Zhibian acupoints with 3-cun stainless steel fine needles of NO.28.Routine sterilization with 75%alcohol and cotton ball on the acupoints.Acupuncture other acupoints with 1.5 cun needles of NO.30.After getting qi,use mild reinforcing and attenuating method. Maintain the needles for 30 minutes.Acupuncture once daily and 5 times a week. One week forms one treatment period and there're two treatment periods in total.The Simple Chinese Herbal Medicine Group only adopts the modified Angelicae Pubescentis and Loranthi Decoction.The Simple Acupuncture Group takes the Foot Taiyang Prescription:Kidney-shu,Large Intestine-shu, Huantiao, Zhibian, Weizhong, Yanglingquan, Chengshan and Kunlun. Foot Shaoyang Prescription:Kidney-shu, Large Intestine-shu, Huantiao, Yanglingquan, Xuanzhong and Qiuxu. The treatment method is the same as the Acupuncture and Chinese Herbal Medicine Group.After two treatment periods, evaluate the curative effect of each group before and after treatment with Ischiodynia Curative Effect Standard Form,Movement Pain Assessment Form and McGill Pain Scale.To ensure consistency and standardization,the same person conducts the assessment.Data processing methods:statistical method:χ2 test for the classified material,the Ridit analysis for two samples of the ranked data,t test for the average comparison,pair t test for self AP comparison and one-way ANOVA deviation analysis for interclass comparison. Use Epi Data 3.01 software to set up a database and use software SPSS 17.0 to analyze statistically and work out the statistical graphs with the software Graph Pad Prism 4.03.ResultsThere're 90 qualified subjects with 30 in the Acupuncture and Chinese Herbal Medicine Group (male 14,female 16),the Simple Chinese Herbal Medicine Group(Male 12,female 18)and the Simple Acupuncture Group (Male 16,female 14) whose average age are 48.4,50.6 and 51.5 respectively.Comparing the age, gender,movement pain score,McGill Pain Score before treatment,the author finds the difference has no significant difference (P>0.05) It indicates there's basic equilibrium in the three groups'baseline conditions before treatment and there's comparability.After two periods'treatment,the comparison of the curative effect shows:The Acupuncture and Chinese Herbal Medicine Group:full recovery rate 30%, excellence rate:66.67%and the total effective rate 96.67%.The Simple Chinese Herbal Medicine Group:full recovery rate 6.67%,excellence rate:33.33%and the total effective rate 40%.The Simple Acupuncture Group:full recovery rate 10.00%,excellence rate:26.67%and the total effective rate 36.67%.Analyze each group's curative effect by Ridit and the author finds that there's significant difference(P=0.012,P<0.05)between the Acupuncture and Chinese Herbal Medicine Group and the Simple Chinese Herbal Medicine Group;comparing the Acupuncture and Chinese Herbal Medicine Group and the Simple Acupuncture Group shows significant difference(P=0.031,P<0.05);comparing the Simple Chinese Herbal Medicine Group and the Simple Acupuncture Group shows no significant difference(P=0.716,P>0.05);the Acupuncture and Chinese Herbal Medicine Group:highly significant difference (P<0.01)is found in movement pain assessment before and after treatment. Simple Acupuncture Group: significant difference(P<0.05) is found in movement pain assessment before and after treatment.The Simple Chinese Herbal Medicine Group:no significant difference is found in movement pain assessment before and after treatment. Comparing the Acupuncture and Chinese Herbal Medicine Group and the Simple Chinese Herbal Medicine Group,the author finds the descending degree of pain assessment scores has significant difference(P<0.05). Comparing the descending degree of McGill Pain Scale scores,the author finds there's significant difference between Acupuncture and Chinese Herbal Medicine Group and the Simple Chinese Herbal Medicine Group (P<0.05) in PRI.There's no significant difference in three groups'descending degree of PRI Scores. Regarding the descending degree of PRI total scores, there's significant difference among the three groups (P<0.05). Regarding the descending degree of VAS score,there's significant difference between the Acupuncture and Chinese Herbal Medicine Group and the Simple Chinese Herbal Medicine Group (P<0.05). Comparing any two groups of the three in the descending degree of PPI scores,the author finds there's no significant difference.Conclusion1. The Acupuncture and Chinese Herbal Medicine Group has better curative effect than the Simple Chinese Herbal Medicine Group and the Simple Acupuncture Group in ischiodynia treatment.2. The Movement Pain Assessment score shows the descending degree of the Acupuncture and Chinese Herbal Medicine Group is superior to that of the Simple Chinese Herbal Medicine Group, but there's no significant difference with the Simple Acupuncture Group.3. The McGill Pain Scale score comparison shows there's no significant difference among the three groups'PRI sentiment scores. The Acupuncture and Chinese Herbal Medicine Group can lower the PRI total score more significantly than the other two groups.4. The VAS score shows the Acupuncture and Chinese Herbal Medicine Group and the Simple Chinese Herbal Medicine Group can effectively lower VAS score. There's no significant difference compared with the Simple Acupuncture Group.5. The PPI score shows there's no significant difference in comparing any two of the three groups.
