Objective To investigate stigma,coping modes,stoma adjustment and quality of life in colorectal patients with permanent stoma.To analysis the correlation between stigma and coping modes,stoma adjustment and quality of life.To analysis the influencing factors of stigma,stoma adaptation,and quality of life,to provide evidence for clinical nursing intervention.Methods Totally 202 patients from six grade 3 hospitals of midland with permanent colostomy were recruited by cross-sectional survey.They were investigated with general information questionnaire, social impact scale(SIS), medical coping modes questionnaire(MCMQ),the ostomy adjustment inventory(OAI-20) and stoma quality of life scale(Stoma-QOL) to get their stigma level,coping modes,stoma adjustment level and quality of life.Results The patients’ SIS score was(60.7±10.4).The facing,avoiding and yielding dimensions scores were(19.2±4.2),(10.5±3.4),(16.6±2.6).The OAI-20 total score was(53.4±16.1). The stoma-QOL total score was(44.4±12.8).The SIS total score was negatively related to the score of facing dimension(r=-0.18,P<0.01),positively related to the score of avoiding dimension(r=0.41,P<0.01).The SIS total score and factor scores were negatively related to the total score of OAI-20(r=-0.22~-0.54 P<0.01)and stoma-QOL total score(r=-0.39~-0.55,P<0.01).The multiple regression analysis revealed the degree of communicate with medical staff had significant negatively impact on stigma(β=-2.95), more women than men take the yield of coping,the degree of communicate with medical staff had significant negatively impact on yield of coping(β =-0.24),age,social isolation and financial insecurity had significant positively impact on yield of coping(β =0.19~0.42);the degree of self-care stoma,social isolation, financial insecurity had significant negatively impact on stoma adaptation(β=-1.07~-2.11), the degree of communicate with medical staff had significant positively impact on stoma adjustment(β=2.39),the degree of self-care stoma,internal shame and financial insecurity had significant negatively impact on stoma-QOL(β=-1.03~-2.32).Conclusions This study shows that the patients’ OAI-20 and stoma-QOL scores are at the middle level, interventions should be taken for the main problems and its influencing factors. The patients tend to take negatively coping modes like avoiding and yielding,should be given specific guidance for taking effective coping modes,The patients’ stigma is closely related to coping modes,stoma adaptation and quality of life.It is suggested to pay more attention to the patients’ stigma.
