Pilot projects to introduce a new-type cooperative medical system in rural and pastoral areas(INCMS) were launched in 2003 in China and Inner Mongolia in 2004. The new system played an important role in relieving poverty caused by disease. However, as a medical security system in the rural and pastoral areas, it is only the beginning. Whether INCMS can achieve sustainable development become the core issue to be solved.This paper studied on the nature, characteristics and development of INCMS, analized the overall operation and typical performance on agriculture, animal husbandry, farming and animal husbandry, forestry, border, urban and rural areas. The author points out issues constraints the sustainable development of INCMS, including legal, finance, compensation, supervision, medical services etc. Combined with deepening of medical health restructure, the paper also give some new ideas and new initiatives for the government and relevant departments in decision-making.In the development concept, the paper proposed to grasp the trend of unified urban and rural health insurance, foster the "common belief" to promot sustainable development of INCMS. Meanwhile, to strengthen the public health work as implementing the strategy of revitalizing China through science and education and vigoriousely advocate the implementation of "health enriching strategy";In the general idea, the paper suggests to master the national policy of deepening medical reform. In accordance with local conditions, finance and situation, establish diversified, multi-level, flexible and progressive rural and pastoral medical security system which is mainly led by the State Council and tanken charge by regional government.In specific policies, the paper suggests that Inner Mongolia should make relating polices in advance, improve financing and compensation mechanism, improve medical and health service system in rural and pastoral areas, strengthen the tripartite coordination of supply, management and requirement, strengthen medical assistance, solve the health insurance issues for the elderly, children and special groups such as the floating population to promote the reform of urban and rural development, etc.
