Study On The Curricula Design And Role Of Oncology Clinical Nurse Specialists

Research purposesIt was widely acknowledged that Clinical Nurse Specialists (CNS) could provide advanced nursing practice and quality care. In some developed western country such as USA,Canada, it has become a systemically,wholly and mature institution after development of about a century. However, institution of CNS in China is still its infancy, therefore,the introduction of CNS will promote the development of specialist nursing in our country to a higher level. The purpose of this study was to establish the curricula design as well as role of oncology CNS through a variety of methods; to provide outline for CNS education in the coming future, to realize systemization and standardization of oncology CNS; to expedite the training of CNS and nursing level of China.ObjectUsing the Delphi method, a lot of 35 experts in different nursing field were consulted. These experts come from some medical universities and hospitals in Beijing, Tianjin and Shanghai. Age range is from 35 to 68,average age is 42.6±7.81(SD), Working years of these experts from 16 to 45, the mean is 24.5±5.38(SD). All the experts have high technical posts.MethodsApplication of the literature analysis, theoretical analysis, expert consultation to explore the theory basis and basic principle for curricula design as well as role of oncology CNS; selects various curriculums as well as role of oncology CNS By Delphi method and weighs the importance; use cronbach alpha coefficient, correlation coefficient, etc. System reliability and validity; SPSS 11.5 application Excel 2003 and statistical software for data processing.Results1. Expert Group on a reasonable selection, the high credibility of the findingsFrom the qualitative analysis of experts'basic situations (Specialty correlating, Age structure, Post and Technical title, etc.)and the quantitative analysis of experts'authority(Authority Coefficient of experts group were 0.8677),the article shows that the expert group is more authoritative, representative, and hierarchical; the questionnaires of the two rounds of consultation are collected 94.29% and 96.97% respectively, and there are 42.43% and 28.13% of experts who give suggestions and comments in the two rounds of consultations respectively, all these show that the experts are more enthusiastic and concerned about the subject; the experts'coordination coefficients about the survey are 0.386 and 0.393(P<0.001) respectively, which shows that the survey result is good. Therefore, the predictions of experts are set up at the practical experience and theory on the foundation, the higher the credibility, the findings desirable.2. Curricula design results show that:5 courses are setted which has a average score higher than 4.5, namely:cancer nursing, cancer psychology, cancer nutrition, treatment and palliative care, hospice care. The setting of courses shows that the experts take more attention to the knowledge and techniques of oncology nursing. They treat the professional courses which has a closed relationship with clinical practice are most important, the other courses less important.3. Filtrate Index and Set Weight of ItemReference to the role of foreign CNS, combined with china's national conditions and nursing staff characteristics, the screening criterion is greater than 4.0, CV is less than 0.25.This study is in odder to form the role system of oncology CNS through two rounds of consulting experts and combining with the advice of research group members, the role system of oncology CNS includes 6 roles and 41 jobs associated with it.Various roles and job contents are set weight by the expert scoring. The separate weight of various roles and job contents is to calculate and normalize the average of each role and job content. Combination weight is calculated by product method. The weight value of eight lectures formed finally are separately 0.1723,0.1645,0.1698,0.1667,0.1688 and 0.1578.4. Evaluation system has good reliability and validitySystem by analyzing the reliability of the experts, surface validity, content validity, etc. showed that the system has good reliability and validity. In the result, Cronbach alpha is 0.790, reliability is good. The Content Validity was proved to be good by logical analysis. The Correlation Coefficient between the roles and each job content ranged from 0.514-0.821,The system had a better reliability and construct validity.Conclusion1. Establishment of the oncology CNS curricula:a total of 24 courses, the professional courses with closed clinical practice such as cancer nursing, cancer psychology, cancer nutrition, treatment and palliative care, hospice care are most important, courses such as nursing research methodology as well as English, computer, literature search, nursing education, etc. are less important.2. Establishment of the Evaluation System of oncology CNS:including clinical practitioner, consultant, educator, manager, research and coordinator and 41 job content associated with it.Originality in this study1. Defined the curricula design and role of Oncology CNS, the domestic has not been reported.2. Provided outline for CNS education in the coming future, which is in turn to enhance the development of the oncology CNS education.
