Objectives: The Clinical Nurse Specialist (CNS) is a nurse who through study and supervised practice at the graduate level (master's or doctorate) has become expert in a defined area of knowledge and practice in a selected clinical area of nursing. The CNS is introduced as a new nursing model that will impel the advanced development of specialized nursing. But there are no a kind of CNS education model can be used for every specialized area in China. The study was aiming at finding out the expectation and demand in the roles of CNS among the clinic nurse, nurse manager and doctor, and finding out the opinions about admission, selecting method, training form, schooling time and curricula design of the CNS among the clinic nurse, nurse manager and nursing teacher. Moreover, it was also for clarifying the roles of the CNS, putting forward a set of training scheme, and then providing outline for CNS education in the coming future.Subject: The subject of the pilot study consisted of 20 clinical nurses, 10 nurse managers and 10 doctors from a hospitals at grade III at Guangzhou city and 10 faculty at nursing department from a medical university. The formal subject consisted of 315 clinical nurses, 153 nurse managers and 94 doctors from 6 hospitals at grade II & III at the Guangzhou areas and 47 faculty at nursing department from 4 medical universities. 565 clinical nurses, nurse managers and doctors took the questionnaires on the role functions of the CNS, while 518 clinical nurses, nurse managers and nursing teachers took the questionnaires on the qualification, selecting methods, training, schooling6time and course arrangement for the CNS trainees. At the rate of 1:3 selected the doctors and the clinical nurses respectively by stratified random sampling, whereas the amount of the nurse managers and the nursing teachers were so small that they were selected by cluster sampling.Methods: The study was in the form of questionnaires. In the pilot study, the content reliability was evaluated by experts; the internal consistency of questionnaire was examined by calculating Cronbach alpha; selection of items in the CNS role questionnaire was base on correlation analysis; construct validity was evaluated by factor analysis as well as correlation analysis. In the formal study, construct validity was evaluated by factor analysis as well as correlation analysis; the internal consistency of questionnaire was examined by calculating Cronbach alpha; describing statistics was used to examine demographic characteristic, admission, selection method, length of schooling and training methods; mean and SD was used to describe the subjects' perception of the importance for the nurse specialist role components and courses; ONEWAY ANOVA, Kruskal Wallis and Independent sample t test were used to analyze group differences in perception of the importance of the nurse specialist role components and courses; mean and percentages were used to select courses.Results:1 The questionnaire, which was based on related authoritative literatures, was revised according to specialists' opinion and result of the pilot study. The result showed that the Cronbach alpha of the role functions and the curricula design of the CNS > 0.90; The questionnaire on the role of the CNS had a better construct validity.2 The research of the role of the CNS indicated: 84.32% of the subjects approved the importance on the clinical practice function, mean (4.34?.41),7ranking in the fist place among the five functions of CNS; the research function rank in the next place, with the proportion of 81.93%, mean (4.29?.50); the education one with the rate of 68.17% and mean (4.12?.45) followed; then it is the management function with the proportion of 59.07% and mean (3.98?.58); and in the last place the consultant function with the rate of 37.64% and mean (3.62?.64) was.
