Objectives:To investigate the cognition,attitude and behavior of clinical nursing toward incidents repors, explore of the nurses toward incidents repors and analyse the relator influencing factor affecting incidents repors.Methods:Self-design a questionnaire on the basis of literature review, expert interview and the qualitative interviews of 12 clinical nurses in 2 non-profitable 3-A-Class hospitals in shanghai. From September to November in 2009, Total 450 clinical nurses in 4 non-profitable 3-A-Class hospitals and 3 non-profitable 2-A-Class hospitals in shanghai were surveyed by a self-designed questionnaire about the awareness,attitude,behavior of report and a report culture of hospital. The data obtained of using the SPSS 13.0 statistical software to establish databases and statistically process. the results of survey combined with the actual situation of the reporting system of clinical incident events in Shanghai hospital to discuss the intervened countermeasures and suggestions.Results:①Total 450 copies of the questionnaire were delivered to the subjects.411 valid questionnaires were received (91.3%response rate).②The feeling of nurses on a report culture of hospital was below the medium level (2.99±0.32 points), the attitude of nurses to the competent departments and the effectiveness to timely feedback are in the middle (3.74±0.43,3.34±0.52), and to the non-punitive environment and the confidentiality are negative(2.39±0.52,1.51±0.50).③The correct rate of the awareness of nurses on incidents repors was 63.7%, of which the master degree on awareness of the incidents repors'importance was the best, the cognitive on general knowledge of incidents repors was the weakest, in particular, on the report's time frame and the concept of difference.④The attitude of nurses on incidents repors is in the middle (3.23±0.39 points), of which on the factors of hindering the report,this dimension showed a negative attitude (2.44±0.70 points), the attitude of the factors of promoting the report showed neutrality attitudes toward this dimension (3.85±0.58 points).⑤The behavior of nurses on incidents repors, of which the ICU group,3~5 years group and 10~19 years group reported the lowest incidence rate, and the undergraduate group,the nurse-in-charge group and above the two group reported the highest rate, and during the past 12 months, the secondary group encountered, but not report clinical incident events has the highest incidence.Conclusions:Nurses are the main executive officers of care safety monitoring system. According to nurses'knowledge, belief and practice in incidents repors and the hospital status of the inform-cultural atmosphere, with the hospital specific circumstances, to explore and build feasible, effective reporting and monitoring system on clinical incident events, and to take a diversified approach would be to prevent and reduce clinical incident events, to enhance the quality of care safety management has a positive effect.
