Objective: To calculate healthy life expectancy (HALE) which is comparative across populations and analyze health related factors among adults in Chengdu.Methods:A multi-stage sampling was used to obtain 1,800 permanent residents aged≥18 in three districts in Chengdu. A questionnaire was administered to gather the information of self-reported health and health related factors. Cut point shift bias of self-reported health was calibrated using CHOPIT model. Sullivan method was applied to calculate adults' healthy life expectancy. Classification tree model was used to explore health-related factors.Results: Respondents tended to report "good" or "very good" in general health and 8 health domains.The rate of reporting "bad" or "very bad" in general health was 10.6%. Less than 3% respondents reported "extreme" difficulty in 8 health domains, which showed "top effect". Variations in health vignettes ratings were found across individuals. CHOPIT model results showed that factors such as region, age, income affected the cut points. After correcting cut-point shift bias, the health rates in all age groups decreased with ages. The healthy life expectancy of adults in Chengdu aged at 20-was 39.62 years, and aged 60-was 11.93 years among both genders. However, females'HALEs were higher than males' at all age groups, but females'life expectancy lost and the proportion of DLE/LE were higher than that of males because of non-fatal health outcome.The rate of smoking was 32.4%, and the rate of regular drinking was 13.2% among adults in Chengdu. Male and people aged at 45-tended to be heavy smoker and alcohol consumer. The rate of active exercise of adults in Chengdu was 17.1%. Fruit intake amount of adults in Chengdu was 0.85±0.69 standard statistical unit per day.23.1% respondents took physical examination in the past 12 months, and 23.4% measured blood lipids regularly. The prevalence of chronic diseases was 18.9%, and higher prevalence was found in female, the elderly and adults in Jinniu district.46.7% patients treated chronic diseases regularly. Two-week prevalence of adults in Chengdu was 171.1‰, and higher prevalence was found in the people aged over 60 and the adults in Qingbaijiang district.74.2% of patients with two-week illness were treated. The incidence rate of serious injury among adults in Chengdu was 109.3%o. Results from Classification tree model analysis showed that adults' health was mainly affected by chronic illness, two-week illness, and physical examination, the relative importance of which were 100.0%,78.7% and 63.8%.Conclusion:"Top effect" was found in self-rated health among adults in Chengdu. The use of CHOPIT model can correct it. Healthy life expectancy of adults in Chengdu was low. Smoking and drinking, low rate of active exercise, low percentage of physical examination and regularly measure blood lipids, and the low percentage of regularly treatment of chronic disease needed be concerned. Health promotion in Chengdu should mainly focus on the prevention and treat of chronic diseases and two-week illness, as well as the encouragement of people's physical examination.
