Objective:To observe the changes of serum adiponectin (ADP), homocysteine (HCY),cholesterol (Chol), triglyceride (TG), high density lipoprotein (HDL) and low density lipoprotein (LDL) in patients with acute cerebral infarction.To research their relevance, the relationship of the severity of infarction and the clinical significance in acute cerebral infarction. The ADP's role in acute cerebral infarction and its possible pathophysiological mechanisms are also be explored.Methods:Select 28 patients with cerebral infarction for CI group (cerebrol infarction ), and 20 cases of healthy acted as control group. Both were comparable in age, gender composition. The enzyme-linked immuno- sorbent assay (ELISA method) was used to detect the level of serum ADP ,and the Enzymatic colorimetric assay was used to detect the level of serum HCY and lipid. And observe the changes of ADP,HCY and lipid in patients with acute cerebral infarction. The CI group were given head CT and / or MRI examination. The infarct volume was calculated by pullicino. According to the size of infarct volume patients were divided into Lacunar infarction group (maximum size of infarction in diameter≤15mm) and non-lacunar infarction group (the largest area of infarction in diameter>15mm).The results are statisticed by statistical software SPSS 12.0. CI group and control group use two independent samples t-test. The changes of serum ADP, HCY and blood lipid levels use paired t-test respectively. The relationships between serum ADP and infarct volume, HCY use linear correlation analysis. P<0.05 means the difference has statistical signi- ficance. Results:1.The mean serum ADP level in the patients with cerebral infarction within 48 hours was significantly lower than in control group (P<0.01), while the mean HCY level was significantly higher than in control group (P<0.01). Compared with the control group, the mean level of serum Chol, TG, HDL, LDL did not have significant difference. The mean level of serum ADP, HCY, Chol, TG, HDL and LDL in the patients with cerebral infarction at the 10th day did not have significant difference compared with the control group.2.The changes of serum ADP, HCY in CI group in the course of the first 10 days and within 48 hours were significant difference (P<0.05). The level of serum ADP significantly increased, while HCY significantly reduced. However,there was no changes in lipid levels.3.The mean serum level of ADP of lacunar infarction group was significantly higher than non-lacunar infarction group (P<0.05), while the mean serum level of HCY and lipid had no significant difference.The mean serum ADP was negatively correlated with infarct volume.4.The mean level of serum ADP in the patients with cerebral infarction was not found correlation with HCY, either with Chol, TG, HDL and LDL.Conclusion:1.The mean serum ADP level in the patients with cerebral infarction within 48 hours was significantly lower than in control group, while the mean HCY level was significantly higher. The mean serum Chol, TG, HDL, LDL level did not have significant change, which indicated ADP and HCY might be involved in the occurrence and the development of acute cerebral infarction, but lipid may not.2.The mean serum ADP level in the patients with cerebral infarction at the 10th day significantly increased compared with within 48 hours, and the mean serum HCY level decreased significantly, suggesting that the change of serum ADP, CRP is closely related to the occurrence and development of acute cerebral infarction.3.The mean serum level of ADP of lacunar infarction group was significantly higher than non-lacunar infarction group. The mean serum level of ADP was negatively correlated with infarct volume, which can reflect the severity of cerebral infarction. The detection of serum ADP can help us to judge the severity of patients with acute cerebral infarction.And the treatment to raise the level of serum ADP may be uesful to improve the prognosis of acute cerebral infarction.4.The mean level of serum ADP in the patients with cerebral infarction was not found correlation with HCY, either with Chol, TG, HDL and LDL, which indicated that their role of acute cerebral infarction and / or related mechanism may be different.
