The Clinical Research Of Yiqi Wenyang Huoxue Tongluo Treatment To Prevent And Treat Oxaliplatin-induced Peripheral Neurotoxicity In Colorectal Cancer

ObjectiveThis controlled study was to compared the incidence of peripheral neurotoxicity,the time of accumulative peripheral neurotoxicity after chemotherapy and the maintenance time of accumulative peripheral neurotoxicity,other side effects of chemotherapy and quality of life of the program with oxaliplatin plus Yiqi Wenyang Huoxue Tongluo treatment and the program with oxaliplatin alone in advanced colorectal cancer,to explore the feasibility of Yiqi Wenyang Huoxue Tongluo treatment of oxaliplatin-induced peripheral neurotoxicity,and the application value in the oxaliplatin-induced peripheral neurotoxicity.Methods60 patients with qualified advanced colorectal cancers were randomly divided into 2 groups.The test group(30 cases)of "Tongluo fang" internal,"Tongluo tang" external was combined with mFOLFOX4 regimen.Specific chemotherapy for oxaliplatin 85mg/m~2 intravenous drip for 2h(1st day),CF 200mg/m~2 intravenous drip for 2h(1st,2nd days),5-FU 2.0g/m~2 continuous infusion for 48h.Every 14 days for a cycle.And since one day of the beginning of chemotherapy to the fourth day for taking "Tongluo fang" five days,twice daily. At the same time,all patients washed and soaked upper limbs by "Tongluo tang"warmly,20 minutes every time,twice daily.The control group(30 cases)was treated with mFOLFOX4 regimen alone.As 4 cycles chemotherapy for an observation period,that means the use of mFOLFOX4 program at least 4 cycles,the internal use of "Tongluo fang" at least 20 times,the external use of"Tongluo tang" at least 20 days.Results60 patients were completed at least 4 cycles treatment,53 patients were completed 8 cycles treatment,43 patients were completed 12 cycles treatment.1)In the incidence rates of peripheral neurotoxicity,the test group was better than the control group(23.46%vs 48.0%,P＜0.05),the difference was statistically significant,especially in the incidence rates of accumulative peripheral neurotoxicity the test group after 8 cycles was lower than the control group(28.57%vs 60.0%,P＜0.05),the test group after 12 cycles was significantly lower than the control group(39.13%vs 80.0%,P＜0.05),the difference was statistically significant;2)In aspect of the time of accumulative peripheral neurotoxicity after chemotherapy,the test group and the control group were respectively(139.53days±41.39daysvs124.65days±46.37days, P＜0.05),the difference was statistically significant.In the maintenance time of accumulative peripheral neurotoxicity after stopping drugs,the test group and the control group were respectively(35.73days±32.46days vs 116.30days±88.04days,P＜0.05),the difference was statistically significant;3)In other side effects of chemotherapy,the test group and the control group have no significant differences(P＞0.05),but the test group were significantly different in neutropenia and hemoglobin(P＜0.05)by comparing with the control group;4)In the quality of life,the test group was better than the control group,the difference was statistically significant(P＜0.05).ConclusionYiqi Wenyang Huoxue Tongluo treatment has a certain degree of clinical efficacy in prevention and treatment to oxaliplatin-induced peripheral neurotoxicity,to reduce the incidence rates of peripheral neurotoxicity,specifically in accumulative peripheral neurotoxicity,extend the time of accumulative peripheral neurotoxicity after chemotherapy and reduce the maintenance time.Yiqi Wenyang Huoxue Tongluo treatment improved the quality of life in patients with advanced colorectal cancer and has better security,non-drug-related adverse reactions.And it has a certain advantages in the prevention and treatment programs with oxaliplatin-induced accumulative peripheral neurotoxicity. Clinical research need to expand further samples,multi-center,randomized observation.
