A Study Of Psychosocial Factors On Bronchial Asthma With Depression

Objective:To investigate the depression of bronchial asthma patients;study the relation between psychosocial factors and bronchial asthma with depression:find out the key factors of affecting bronchial asthma with depression,probe into the psychosocial mechanism of bronchial asthma with depression:in order to provide the academic data for instituting integrative mental intervention on asthma in future,thereby prevent,treat,and control asthma effectively,improve life quality of asthma patients,and debase the incidence and mortality rate of asthma.Methods:A total of 216 cases of hospitalized bronchial asthma patients were investigated by schedule table SDS,NEO-PI-R.ATQ,DAS.TCSQ.SSRS,at the same time.measured their Peak Expiratory Flow(PEF).With the SPSS 11.5 and the Amos4.0.analysis of variance,t-test.Pearson correlation analysis,multivariable stepwise regression analysis and path analysis were carried out for the collected data.Results:In 216 cases of bronchial asthma patients.106 cases of patients had depression emotion.including 60 cases of mild depression.32 cases of medium depression and 14 cases of severe depression:pulmonary function of bronchial asthma with depression group cases were significant lower than no-depression group cases.There are significant differences in depression extents among bronchial asthma patients with different education levels,different occupations,different states of asthma and different incomes:there were significant differences in personality,traits between depression group cases and non-depression group cases, neuroticism,extraversion and conscientiousness were significantly related to depression;personality traits of bronchial asthma patients were related significantly to their recognition,coping style and their social support.There were significant differences in automatic thinking and dysfunctional attitudes between the depression group cases and non-depression group cases,and the automatic thinking and dysfunctional attitudes were also related to depression significantly;coping style was significantly different between depression group cases and non-depression group cases,and was related to depression extents.There were significant differences in total score of social supports,objective support and availability of support between depression group cases and non-depression group cases,and they were correlative with depression extents.In paths influencing depression,there were 6 direct and 7 indirect paths,neuroticism was the most powerful affecting factor for patients with bronchial asthma with depression,and besides affecting depression directly,it affected indirectly by objective support,negative coping,automatic thinking and dysfunctional attitude.Conclusion:1.The bronchial asthma patients investigated who have depression account to 49.1%;there are significant differences in depression extents among bronchial asthma patients with different education levels,different occupations. different states of asthma and different incomes.2.Pulmonary function of bronchial asthma patients with depression was lower significantly than no-depression group cases.3.Bronchial asthma patients have their unique personality traits:neuroticism and conscientiousness were key factors of affecting bronchial asthma with depression:personality traits of bronchial asthma affect their recognition,coping style,and social support.4.There were further automatic thinking and dysfunctional attitudes on bronchial asthma depression patients.5.Bronchial asthma with depression patients tend to adopt negative coping style,and no-depression bronchial asthma patients tend to adopt active coping style:the coping style is the important mesomeric factor between personality and depression of bronchial asthma patient.6. Social support got by bronchial asthma patients with depression are fewer than no-depression bronchial asthma patients:social support can play a role of relieving depression in bronchial asthma patients:social support can affect coping style in asthma patients.7,Among personality,traits,recognition,coping style,social support and income,personality is the most key factor for predicting depression in bronchial asthma.8,We suggest that we should strengthen intervention for depression in future integrative intervention in asthma.
