Slective Studying Of The Treatment Of The Type-MasonⅢ Fractures Of The Radial Head

Objective :To assess the early results and safety of excision and replacement of the radial head for the treatment of Type-Masonâ…¢fractures with the pain score of numeric rating scale, restoration of elbow function, deformity improvement, observation of X-ray and health economics, etc. in order to provide some proves for the patients to choose.Methods: This article reviewed the knowledge of the radial head fracture in both ancient literature and modern medical research systematically. Summarizing the etiology, pathology, medicine of FDR in TCM;Describing the Treatment methods and progress; Suming up the development of the anatomy, reason of injury, research of biological mechanism, diagnosis and treatment. 30 patients who suffering from Type-Masonâ…¢fractures of the radial head were collected. We divide these patients into two groups. One group (15cases) were treated by excision of the radial head, the other (15cases) were treated by replacement of the radial head. Both group were observed 18 weeks. Every patient was evaluated by Broberg and Morrey elbow evaluate form ,X-ray evaluate form and Medical Economics Evaluation.Results: Through clinical research, the result shows that replacement group is better in the pain score of NRS 1^st day and 14^th day after operation (P<0.05), But there was no statistical significance in the pain score of NRS since 14 days after operation; The result of the function comparison between two groups is the replacement group is better than excision group from 1^st day to 18^th week after operation (P<0.01, 14^th day, P<0.05); The X-ray image of the replacement group after operation is obviously better than excision group (P<0.01); The replacement group is better than excision group in the complications; Last, through the Medical Economics Evaluation, we found that the replacement-group is high superior than the excision-group (P< 0.01) .Conclusion: Replacement of the radial head not only can provide early pain relief and reduce complications but also improve the function of the elbow evidently. However, Excision of the radial head with functional exercise early is still a simple, economic and reliable method, easier accepted by the economic difficulties and poor physical condition of elderly patients.
