Objective:To analyze the clinical effect of bone setting manipulation combined with elastic intramedullary nail fixation on O’Brien type II children’s radial neck fracture,and to explore the application of bone setting manipulation in minimally invasive treatment of children’s radial neck fracture.Methods:O’Brien type II children’s radial neck fracture from January 2016 to December 2018,were analyzed retrospectively.According to the differences of operation methods,the patients who met the inclusion criteria were divided into two groups:manual combination group(Orthopedic manipulation combined with elastic intramedullary nail internal reduction and fixation)and simple group(simple intramedullary nail reduction and fixation).According to the relevant clinical data of the two groups collected by the inpatient case system and using statistical software to analyze the collected data,respectively observe the reduction situation after treatment,including the length of operation,the times of fluoroscopy,the clinical healing time of fracture,and the evaluation of elbow joint function after operation in the combination group and the simple group.Results:A total of 56 children with O’Brien II were included in the study,with an average age of 10.0years,including 34 males,22 females,25 on the left and 31 on the right.The follow-up time was 6-24 months.The average follow-up time was 15 months.All cases healed clinically and no complications were found.There was no significant difference between the two groups in fracture healing time(42.63±1.75 d vs42.97±1.72d),P>0.05;According to metazeau’s later evaluation standard,the results showed that the excellent and good rate of the combined manipulation group was 96.7%,including 24 excellent cases,5good cases,1 middle case,0 poor case.Excellent and good rate of the simple group was 92.3%,including 15 excellent cases,9 good cases,2 middle cases and 0 poor case.The difference between the two groups was not statistically significant(P>0.05).The operation time of combined manipulation group was faster than that of simple group(30.03±4.16minvs38.08±2.67min)and the difference was statistically significant(P<0.05);In terms of the times of intraoperative fluoroscopy,there was much less comparison between the combined manipulation group and the simple group(8.53±3.72 vs 11.96±3.54),which was statistically significant(P<0.05);According to the metazeau reduction standard,the difference between the two groups was statistically significant(P<0.05).In the combined manipulation group,19 cases were good,10 cases were good,1 case was middle and 0 case was poor.In the simple group,9 cases were good,14 cases were good,3 cases were medium,and 0 case was poor,The combination group is better than the simple group.Conclusion:Traditional Chinese medicine bone setting technique can improve the reduction effect of elastic intramedullary nail in O’Brien II type children’s radial neck fracture.It can shorten the operation time,reduce the radiation exposure of doctors and patients,and no related complications and adverse factors affecting fracture healing were observed during the follow up,which is in line with the minimally invasive treatment of the combination of Chinese and Western Medicine,and is worthy of clinical promotion.
