Objective: To investigate the situation of self-management behaviors, coping style, and health belief in patients with type 2 diabetes mellitus. To explore the related factors of self-management behaviors, and analyze the correlation among coping style and health belief and self-management behaviors in these patients.Methods: The subjects were selected through stratified random sampling. Wujialing community of Kaifu district, Pozijie community of Tianxin district were randomly selected from different areas of Changsha. Through the notice and informed consent taken ,211 type 2 diabetes patients were recruited and accepted this survey in two selected communities. Four questionnaires were administrated for the survey: demographic and metabolic control information, medical coping style scale, type 2 diabetes self-management behavior scale and health belief scale. Data were analyzed with software SPSS 13.0 for Windows. Descriptive analysis, independent-samples t-test, one-way ANOVA, Pearson anlysis and Logistic regression were used.Results1.Two hundreds and eleven questionnaires were distributed and 190 were valid with a 90% efficient rate.2.The mean score of patients' coping style was 46.76±5.35 (20-80), with mean score of the confronce factor: 20.17±3.94, the avoidance factor: 15.77±2.66, and the resignation factor: 10.82±2.21. Score of the three dimension were higher than the norm of patients.3.The mean score of health belief were 78.85±9.48(20-100), with'benefit to treatment'getting the highest score and'harm fortreatment'getting the lowest. Patients'health belief was associated with Self-care ability, economic status, training times and in-patient times.4. The mean score of DM patients' self-management behavior were 88.61±18.70 (26-130) , with the dimension'adhere to take medicine'getting the highest score and the dimension'self-monitoring of blood sugar'getting the lowest score.Patients' self-management behavior was associated with economic status, training times, admission times, with or without complication, glycosylated hemoglobin level and fasting blood-sugar level (P<0.05).5.Self-management behavior has a positive correlation with facing factor of coping style and health belief (p<0.01) , and has a negative correlation with resignation factor of coping style (p<0.01) .6.The patient's self-management behaviors was well, their postprandial blood sugar, glycosylated hemoglobin values were lower than the poor self-management behaviors.' There was a significant difference (p<0.01)7. Paying type of medical expenditure, treatment style, occupation, self care status, coping style, health belief, income, glycosylated hemoglobin level, blood pressure control, training times were the significant influent factors on self-management behaviors.Conclusion1.All factors of coping style were higher than the norm. The level of health belief were just satisfactory. Out-hospital self-management level were poor and it is necessary to be improved .2.Complication, economic status, training times, admission times, glycosylated hemoglobin level and fasting blood-sugar level had influence on the self-management behaviors.3.Professional medical experts could improve the patients' out-hospital self-management behaviors by changing the coping style and prompting the health belief level.4.The metabolism control was affected by the self-management behaviors in type 2 diabetes mellitus patients, self-management behaviors was good,the metabolic control was also better.5. Paying type of medical expenditure, treatment style, occupation, elf care status, coping style, health belief, income, glycosylated hemoglobin level, blood pressure control, training times are all the impact factors to self-management behavior of type 2 diabetes Mellitus. More attention should be payed to the patients' coping style and health belief.
