| Objective:A large number of domestic and foreign experiences have shown that hypertension management in community was the effective way.In recent years,some Community Health Service Center through the way such as screening the health records of residents,making a sample survey in high-risk groups to determine the intervention and management of objects for hypertension.And then following-up, intervention and evaluation for the hypertensive patients were carried out by the team of general practitioners and got some preliminary results.How to continue and effectively promote the work of community-based hypertension management, however,has become difficult.In the present study,we explored the effective way of hypertensive management in community,in order to improve the effective interventions and the treatment compliance for the hypertensive patients,and also explore intervention strategies on high-risk groups and general population,more effectively to control the occurrence and development of the hypertension,as well as increase the control rate of blood pressure and improve the patients' life quality.Methods:964 community residents aged 18-year-old and over were inquired by stratified and cluster sampling.They were divided into the intervention group and the control group randomly.Measuring blood pressure,clinical diagnosis,establishing card,following-up,evaluation were carried out in the intervention group through two ways of clinic and community(clinicians' workstation and team of general practitioners),and information software was applied together to complete prevention, cure and management for hypertension.The control group still carried on the original model.The data was analysed by x~2 test.Results:At present,the prevalence rate of hypertension of≥18-year-old residents in Changfeng Community was about 22.8%,significantly higher than the level of national and Shanghai.After intervention,the rate of awareness,control rate of blood pressure,rate of standardized treatment were increased obviously in the intervention group.And also the patient compliance and satisfaction were improved obviously than that in the control group.Conclusion:The prevalence of hypertension in community is still high,the optimizational model of management for hypertension is obviously effective,this model is practicable and deserves to be spread. |
