| Hypertension is one of the common chronic diseases,often coexisting with other cardiovascular risk factors,is an important risk factor of cardiovascular and cerebrovascular diseases,the mortality ranked first.If the blood pressure can not be effectively controlled,can damage the heart,brain,kidney and other important organs,eventually leading to organ failure.The main battlefield of hypertension prevention and treatment in our country is at the community.Therefore,it is of great significance to clarify the situation of hypertension health management at the community and to explore the application effect of general practitioner team model in the primary hypertension health management.Objective:To investigate the prevalence,management rate,knowledge of hypertension,medication compliance and management satisfaction of Zhouling Health Center in Qinhan District,Xi'an.To explore the effect of the team cooperation model of general practitioners on the management of hypertension and chronic diseases at the community,and to provide reference for the standardized management of hypertension at the grass-roots level in the future.Methods:370 patients with essential hypertension who participated in the standardized management of chronic diseases in the community were selected by random sampling method.370 patients with hypertension were divided into experimental group185 and control group 185.The experimental group adopted the general practitioner team cooperation model for hypertension chronic disease management,and the control group adopted the current hypertension management model of Zhouling Health Center.According to the self-designed questionnaire,the control group and the experimental group were investigated,and the compliance,knowledge and management of hypertension were compared between the two groups and 6 months before and after intervention whether there is any difference in meaning,etc.Statistical analysis of the datausing SPSS19.0.Count data is described by rate and composition ratio,chi-square test is used,and measurement data is t test.Test level ?=0.05,p?0.05 was statistically significant.Results:1.The prevalence rate of primary hypertension was 19.73%,which was lower than the national prevalence rate of hypertension 27.9%;the health management rate was39.08%,the standardized management rate was 82.61%,and the blood pressure control rate of the management population was 53.26%,which of which were lower than the health management rate 39.35%,the standardized management rate 83.15% and the blood pressure control rate 62.60% of the primary hypertension patients in Xi'an in 2014.2.Primary hypertension patients are mainly middle-aged and elderly,long course of disease,overweight,central obesity,low education.3.The hypertension knowledge,salt quantity control,medication compliance,self-management ability of patients with primary hypertension are poor,and the satisfaction degree to the current management mode is poor.4.After 6 months of intervention,the body mass index,waist circumference value and blood pressure value of the subjects in the test group and the control group were all lower than those before intervention,and the difference was statistically significant(p<0.05).Health knowledge,health behavior,medication compliance and management evaluation scores of the subjects in the experimental and control groups were significantly higher than those before intervention(p<0.05).After intervention,there was significant difference in health knowledge and management evaluation scores between the experimental group and the control group(p<0.05).Differences in lifestyle and medication compliance scores in the control group were not statistically significant(p>0.05).Conclusion: The hypertension health management rate,standardized management rate and blood pressure control rate of the management population are lower than those of Xi'an City.The team management mode of general practitioners is superior to the traditional management mode.Therefore,Zhouling Health Center can adopt the team cooperation mode with general practitioners as the core to realize the fine management of hypertension patients. |
