Application Of Enteral Nutrition In Preoperative Diet Bowel Preparation For Patientswith Colorectal Cancer Patients

Objectives:To study the feasibility and effectiveness of enteral nutrition formula as preoperative diet bowel preparation for colorectal cancer patients.Methods:48 colorectal cancer patients were randomized into 2 groups.24 patients in the study group(SG) were applied with fresubins from preoperative day 7 to day 4. 24 patients in the control group(CG) were applied with free diet from preoperative day 7 to day 4. From preoperative day 3 to day 1,both groups enter the bowel preparation.SG were continuously provided with fresubins in 3 days.While CG were applied with less semi-liquid slag from preoperative day 3 to day 2 and the clean-liquid diet in preoperative day 1.Meanwhile the mechanical bowel preparation and the drug preparation were the same in two groups. The status of bowel cleansing, liquid volume of the bowel cleansing, times of bowel cleansing, postoperative complications, recovery of bowel sound, nutritional parameters, immunological parameters and inflammatory parameters were observed and analyzed.Results: The status of bowel cleansing were well in both groups. The liquid volume and the times of bowel cleaning in SG were less than those in CG(P<0.05). The incidences of dizziness tiredness and palpitation and the incidences of hunger feeling during the bowel preparation in SG were significantly less than those in CG(P<0.01). Besides the incidence of hypoglycemia in SG before the operation was lower than those in CG(P<0.05). The incidence of e1ectrolyte disturbance in SG was lower than in CG(P<0.05), and the postoperative complications were lower in SG than in CG but the differences were not significant(P>0.05). The changes of nutritional,immunological and inflammatory parameters after the operation had no significant differences as compared with those before he bowel preparation(P>0.05). But the albumin,prealbumin, transferrin, CD3,CD4 and the ratio of CD4 / CD8 were significantly lower,while the level of CD8 and CRP after the operation in CG were significantly higher than those before bowel preparation (P<0.05).Conclusions:The enteral nutrition can be safely and effectively served as the preoperation diet bowel preparation in colorectal cancer patients. It leads to the good result of bowel cleansing, lower liquid volume of the bowel cleaning, less times of bowel cleaning; lower incidences of dizziness, palpitation, tiredness, hunger feeling and hypoglycemia. The nutritional state and immune function can be improved whereas the stress reaction can be reduced with it.
