Objective:This thesis is based on the research result from National 15th tackling key problems"Study on CHS Mode"which was presided by professor Yao and "Study on Performance Appraise and Reward Distribution" presided by professor Zhang, it has absorbed the partial results of the study on "Performance Appraise System of CHS Institution" of H8/S8 and China/UK Urban Health and Poverty Project (UHPP). The objects of this thesis is to establish a personal performance appraise system in CHS institution by defining the performance appraise dimensionality of each kinds of stuff in CHS institution and confirming the weights of each target, it could be a references of establishing a personal performance appraise system in CHS institution for the management of personal performance appraise and relative research department, and be a gist for performing the reformation of reward distribution and system of post wage and performance wage under the background of reforming in both revenues and expenditures, drug zero Variance rate, two-way referral system and national basic health care system.Methods:1. Document study. Consult domestic and foreign literature about the performance theory, performance assessment, health system performance assessment, hospital performance assessment and township health institutions assessment and so on.2. Delphi method. To determine the performance indicators of CHS consult the experts by Delphi inquiry.3. Spot investigation. To collect the performance appraise project of CHS institution in Shanghai, Hangzhou, Chengdu, Wuhan., and doing questionnaire investigation and interview on the stuffs of CHS institution and local inhabitant of those place, then doing a validate for the performance appraise system and it's weights which are result from Delphi. 4. Statistics method. Establishment database and compute the value of standardized indicators and performance index by Microsoft Office Excel2003. Descriptive Statistics, Nonparametric test, Reliability analysis, Correlation analysis are applied by SPSS11.5.Results:1. This thesis has defined the function and content of the task in CHS institution, the personal performance appraise must be a post performance appraise which is changed by different tasks. The stuffs in CHS institution could separate in Clinicians (General practitioners), nurses, public health staff, medical technology staff and managers.2. Personal performance appraises dimensionality include quantity of services, quality of services and satisfaction grade. It could be a three-dimensional solid configuration if we regard those three performances appraises dimensionality as coordinate axis. Performance appraise dimensionality, measuring and target, the methods and tools of performance appraise, the main body and object the circumstance of performance appraise constitutes the performance appraise system.3. The targets of performance appraise have followed the principle of Scientific, practical and feasible, choosing 3 first-degree targets of personal performance appraise of CHS institution by Delphi and confirm 49 second-degree targets according to the different post.4. The results form the 7 central city zone investigation show that the personal performance appraise in CHS have well validity and reliability and can reflect the performance of the stuffs in CHS effectively(Cronbachα=0.7342; VC=0.7368).Conclusion:1. An oriented performance appraise system could perfecting the functions of CHS, improving the efficiency and quality of the service.2. An oriented performance appraise project could guide the reward distribution project, improve the positivity of providing public health service of CHS institution ,keep the energy and efficiency of CHS institution, advance a balanced development of public health and basic medicine. 3. The personal performance appraises system of CHS institution of this research has well validity and reliability, and can reflect the performance of the stuffs in CHS effectively.4. It has to do a further research and be perfected for the performance appraises system with the unceasing establishment and perfecting of the basic health service system. Both of the way to materialize the commonweal of CHS in performance appraise system, the way to contact the results of performance appraise and reward distribution need a further research.
